摘要
目的:探究床旁外周血心肌肌钙蛋白I(cTnI)、氨基末端-B型脑钠肽前体(NT-proBNP)联合检测在早期诊断非心源性急性胸痛(AND)中的应用价值。方法:选择257例非心源性AND患者作为研究对象。按照外周血cTnI、NT-proBNP检测方法的不同将其分为快检组和常规组。为快检组患者床旁快速检测外周血cTnI、NTproBNP的水平,为常规组患者采血后在检验科检测外周血cTnI、NT-proBNP的水平,比较两组患者的检测结果及检测用时,总结用床旁外周血cTnI、NT-proBNP单独检测或联合检测诊断快检组患者病情的效能。结果:两组患者外周血NT-proBNP、cTnI的水平相比,P>0.05。为快检组患者检测外周血NT-proBNP、cTnI水平的用时短于为常规组患者检测这两项指标的用时,P<0.05。与用床旁外周血cTnI或NT-proBNP单独检测相比,用这两项指标联合检测诊断快检组患者非心源性AND的敏感度和特异度均更高,P<0.05。结论:用床旁外周血cTnI、NTproBNP联合检测早期诊断非心源性AND的敏感度和特异度较高,且检测用时较短。
Objective:To investigate the clinical value of the combined detection of cardiac troponin I(cTnI)AND aminoterminal B-type brain natriuretic peptide precursor(NT-probNP)in peripheral blood of patients with noncardiogenic acute chest pain(AND).Methods:257 non-cardiogenic AND patients were selected as subjects.According to the different detection methods of cTnI and NT-probNP in peripheral blood,they were divided into quick detection group and routine group.For the rapid detection group of patients bedside rapid detection of peripheral blood cTnI,NT-probNP level,for the routine group of patients in the clinical laboratory after blood collection detection of peripheral blood cTnI,NT-probNP level,compare the two groups of patients test results and detection time,To summarize the efficacy of bedside peripheral blood cTnI and NT-probNP alone or combined detection in the diagnosis of rapid detection group.Results:Compared with the levels of NT-probNP and cTnI in peripheral blood of two groups,P>0.05.The detection time of nT-probNP and cTnI levels in peripheral blood in the rapid detection group was shorter than that in the conventional group,P<0.05.Compared with bedside peripheral blood cTnI or NTprobNP alone,the sensitivity AND specificity of non-cardiogenic AND were higher in the rapid detection group with these two indicators combined,P<0.05.Conclusion:The combined detection of bedside peripheral blood cTnI AND NT-probNP has high sensitivity AND specificity for the early diagnosis of non-cardiogenic AND,AND the detection time is shorter.
