摘要
目的:观察开窍复聪针刺法治疗突发性耳聋(肝火上炎证)的临床疗效。方法:遵循随机对照原则,将158例肝火上炎证突发性感音神经性耳聋(SSNHL)患者随机均分为观察组(开窍复聪针刺法+常规西药)和对照组(常规西药治疗)各79例,均以10d为1个疗程,持续治疗2个疗程,比较两组临床疗效、中医证候积分、纯音听阈(PTA)值、炎性标志物[可溶性血管细胞粘附因子-1(sVCAM-1)、细胞间粘附因子-1(ICAM-1)]变化,并记录其不良反应发生。结果:观察组与对照组临床有效率分别为87.34%和72.15%,差异具有统计学意义(P<0.05);治疗后,两组中医证候积分、sVCAM-1、ICAM-1均明显降低(P<0.05),且观察组降低幅度更大(P<0.05);治疗前,两组PTA比较无统计学意义,治疗10d、2个疗程后,观察组PTA明显低于对照组(P<0.05),治疗前后组内变化有统计学意义(P<0.05);治疗期间,对照组不良反应总发生率为7.59%,观察组为2.53%,两组之间比较无统计学意义(χ^(2)=2.107,P=0.147)。结论:开窍复聪针刺法治疗肝火上炎证SSNHL可有效改善其症状体征,降低炎症反应,效果确切。
Objective: To study the clinical efficacy of applying Kaiqiao Fucong Acupuncture in the treatment of sudden deafness(liver-fire exuberance syndrome).Methods: Following the principle of randomized control, 158 patients with sudden sensorineural hearing loss(SSNHL) of liver-fire exuberance syndrome were randomly divided into an observation group(Kaiqiao Fucong acupuncture+conventional western medicine) and a controlled group(conventional western medicine treatment),with 79 cases in each group, taking 10 d as a course of treatment, the two groups were continuously treated for 2 courses.The clinical efficacy, TCM syndromes scores, pure tone audiometry(PTA) value and inflammatory markers [soluble vascular cellular adhesion molecule-1(sVCAM-1),intercellular adhesion molecule(ICAM-1)] were compared between the two groups, and the occurrence of adverse reactions were recorded.Results: The clinical effective rate was 87.34% in the observation group and was 72.15% in the controlled group(P<0.05).After treatment, the scores of TCM syndromes, sVCAM-1 and ICAM-1 were significantly reduced in the two groups(P<0.05),and the reductions were greater in the observation group(P<0.05).Before treatment, there was no statistically significant difference in the PTA between the two groups, and the PTA of the observation group after 10 d and 2 courses of treatment was significantly lower than that of the controlled group(P<0.05),and there was a statistically significant change within the group before and after treatment(P<0.05).During treatment, the total incidence rate of adverse reactions was 7.59% in the controlled group and was 2.53% in the observation group(χ^(2)=2.107,P=0.147).Conclusion: Kaiqiao Fucong acupuncture can effectively improve the symptoms and signs and reduce inflammatory response of SSNHL of liver-fire exuberance syndrome, with definite effects.
