铁缺乏伴或不伴贫血症状与老年慢性心力衰竭患者左心室射血分数、运动耐量的关系及对生活质量的影响 被引量:6

Relationships between iron deficiency with or without anemia symptoms and left ventricular ejection frac⁃tion,exercise tolerance in elderly patients with chronic heart failure and their impact on quality of life

摘要 目的探究铁缺乏伴或不伴贫血症状与老年慢性心力衰竭(choric heart failure,CHF)患者左心室射血分数(left ventricular ejection fraction,LVEF)、运动耐量的关系及对生活质量的影响。方法选取邢台市第三医院2017年1月至2019年9月老年CHF患者126例作为研究对象,根据铁缺乏及伴有贫血症状分为铁缺乏伴贫血组(48例)、铁缺乏不伴贫血组(21例)、铁正常无贫血组(57例)。比较3组患者的临床资料,LVEF,运动耐量[6 min步行距离(6-minute walk distance,6-MWT)、最大运动心率、能量代谢当量(metabolic equivalent of task,MET)、峰值耗氧量(maximal oxygen consumption,VO_(2)MAX)],血红蛋白(hemoglbin,Hb),铁代谢指标(铁、铁蛋白)。分析3组患者Hb与铁代谢指标、LVEF、运动耐量的相关性。对比3组患者的生活质量[明尼苏达心力衰竭生活质量问卷调查表(Minnesota living with heart failure questionnaire,MLHFQ)]评分,并比较3组不同生活质量患者Hb、铁、铁蛋白,分析Hb、铁、铁蛋白与生活质量的关系。结果铁缺乏伴贫血组、铁缺乏不伴贫血组、铁正常无贫血组患者Hb与铁、铁蛋白呈正相关(P<0.05);铁缺乏伴贫血组、铁缺乏不伴贫血组患者的Hb、铁、铁蛋白与LVEF、6-MWT、最大运动心率、MET、VO2MAX呈正相关(P<0.05);铁缺乏伴贫血组患者身体领域[(32.20±4.16)分vs.(28.41±3.37)分vs.(25.30±3.02)分,P<0.05]、情绪领域[(19.53±2.24)分vs.(17.10±2.13)分vs.(15.06±1.87)分,P<0.05]、其他领域[(28.76±3.62)分vs.(25.14±3.20)分vs.(21.93±2.84)分,P<0.05]、MLHFQ评分[(80.49±7.61)分vs.(70.65±6.59)分vs.(62.29±5.82)分,P<0.05]高于铁缺乏不伴贫血组、铁正常无贫血组,差异有统计学意义(P<0.05);铁缺乏伴贫血组、铁缺乏不伴贫血组患者Hb、铁、铁蛋白与MLHFQ评分呈负相关(P<0.05)。结论Hb、铁、铁蛋白与老年CHF患者LVEF、运动耐量均存在正相关性,早期明确CHF患者是否存在铁缺乏、贫血,可为临床改善老� Objectives To explore the relationships between iron deficiency with or without anemia symptoms and left ventricular ejection fraction(LVEF),exercise tolerance in elderly patients with chronic heart failure(CHF),and its impact on the quality of life.Methods Totally 126 elderly patients with CHF from January 2017 to September 2019 in The Third Hospital of Xingtai were selected as the research objects.According to iron deficiency and anemia symptoms,they were divided into iron deficiency with anemia group(48 cases),iron deficiency without anemia group(21 cases),and iron normal without anemia group(57 cases).The clinical data,LVEF,exercise tolerance[6-minute walk distance(6-MWT),maximum exercise heart rate,metabolic equivalent of task(MET),maximal oxygen consumption(VO2MAX)],hemoglobin(Hb),and iron metabolism indexes(iron,ferritin)of the 3 groups were compared.The correlations between Hb and iron metabolism index,LVEF,exercise tolerance in the 3 groups were analyzed,and the quality of life[Minnesota living with heart failure questionnaire(MLHFQ)]scores were compared.Hb,iron,ferritin of the three groups of patients with different quality of life were compared,and the relationships between Hb,iron,ferritin and quality of life were analyzed.Results There were positive correlations between Hb and iron,ferritin in iron deficiency with anemia group,iron deficiency without anemia group and iron normal without anemia group(P<0.05).Hb,iron and ferritin positively correlated with LVEF,6-MWT,maximum exercise heart rate,MET and VO2MAX in iron deficiency with anemia group and iron deficiency without anemia group(P<0.05).In iron deficiency with anemia group,the physical field[(32.20±4.16)points vs.(28.41±3.37)points vs.(25.30±3.02)points,P<0.05],emotional field[(19.53±2.24)points vs.(17.10±2.13)points vs.(15.06±1.87)points,P<0.05],other fields[(28.76±3.62)points vs.(25.14±3.20)points vs.(21.93±2.84)points,P<0.05],and MLHFQ scores[(80.49±7.61)points vs.(70.65±6.59)points vs.(62.29±5.82)points,P<0.05]were significantly
