摘要
目的:观察门诊行内镜下高频电凝电切术治疗结肠息肉临床治疗效果。方法:选取江苏省句容市妇幼保健院(以下简称我院)2019年8月-2021年8月中在消化科行镜下结肠息肉冷切除术治疗的70例结肠息肉患者作为对照组,另选择同期在我院门诊行内镜下高频电凝电切术治疗的结肠息肉患者68例作为观察组,观察两组患者各项手术指标、临床疗效、心理状态、日常生活能力及术后并发症情况。结果:经内镜下高频电凝电切术治疗的观察组各项手术指标明显优于对照组,患者术中术后出血量明显更少,卧床时间更短(P<0.05);观察组患者临床治愈率为95.59%(65/68例),对照组患者临床治愈率为94.29%(66/70例),组间差距比较无统计学意义(P>0.05);术后1d采用焦虑评分(HAMA)和心理状态评分(SCL-90)评估患者的心理状态,观察组患者评分更优(P<0.05);使用Barthel指数对两组患者的日常生活能力进行评定,观察组患者平均评分高于对照组;对照组患者术后发生并发症发生率为27.14%(19/70例),观察组患者术后发生并发症发生率为16.17%(11/68例)。结论:根据本文内镜下高频电凝电切术技术在门诊治疗结肠息肉的研究结果可见,该术式能够有效减少患者的手术创伤、减少患者卧床时间,有助于患者的心理状态和术后恢复,并发症更少,可在门诊治疗中推广应用。
Objective:To observe the clinical effect of endoscopic high-frequency electrocoagulation and electrocision in the Outpatient Department in the treatment of colonic polyps.Methods:In the Jurong Maternal and Child Health Hospital in Jiangsu Province(hereinafter referred to as our hospital)from August 2019 to mid-August 2021,70 patients with colonic polyps treated by endoscopic cold resection of colonic polyps in the Digestion Department were selected as the control group,and 68 patients with colonic polyps treated by endoscopic high-frequency electrocoagulation and electrocision in the Outpatient Department of our hospital in the same period were selected as the observation group.The surgical indexes,clinical efficacy,psychological status,daily living ability and postoperative complications of patient in two groups were observed.Results:The surgical indexes of the observation group treated by endoscopic high-frequency electrocoagulation and electrocision were significantly better than those of the control group,the amount of intraoperative and postoperative bleeding was significantly less,and the time in bed was shorter(P<0.05);The clinical cure rate of patients in the observation group was 95.59%(65/68 cases),and that of patients in the control group was 94.29%(66/70 cases).There was no significant difference between the groups(P>0.05).The psychological state of patients was evaluated by anxiety score(HAMA)and psychological state score(SCL-90)one day after operation,and the score of patients in the observation group was better(P<0.05);Barthel index was used to evaluate the activities of daily living of patients in the two groups.The average score of patients in the observation group was higher than that in the control group;the incidence of postoperative complications in the control group was 27.14%(19/70 cases),and the incidence of postoperative complications in the observation group was 16.17%(11/68 cases).Conclusion:According to the research results of endoscopic high-frequency electrocoagulation and electrocis
