摘要
目的探讨磁共振成像(MRI)联合磁共振胰胆管造影(MRCP)在胆管梗阻疾病诊断中的临床应用价值。方法回顾性分析2018年8月至2020年8月医院收治的92例胆管梗阻疾病患者的临床资料,所有患者均行MRI及MRCP检查,以手术病理或镜下逆行胰胆管造影(ERCP)检查结果为金标准,比较单独MRCP与MRI联合MRCP检查的效果。结果经手术病理或ERCP检查显示,病变部位位于肝门及以上段20例,胆总管42例,胰腺段27例,壶腹段3例,MRI联合MRCP检查定位诊断胆管梗阻疾病的准确率高于单独MRCP检查,差异有统计学意义(P<0.05);经手术病理或ERCP检查确诊,良性病变50例,恶性病变42例,MRI联合MRCP检查定性诊断良性病变及恶性病变的准确率均高于单独MRCP检查,差异有统计学意义(P<0.05)。结论相较于单独MRCP检查,MRI联合MRCP检查定位及定性诊断胆管梗阻疾病的准确率均较高。
Objective The clinical application value of magnetic resonance imaging(MRI) combined with magnetic resonance cholangiopancreatography(MRCP) in the diagnosis of bile duct obstruction was explored. Methods The clinical data of 92 patients with bile duct obstruction disease admitted to the hospital from August 2018 to August 2020 were retrospectively analyzed. All patients underwent MRI and MRCP examinations, and the results of surgical pathology or microscopic retrograde cholangiopancreatography(ERCP) was regarded as the gold standard to compare the effects of MRCP alone and MRI combined with MRCP. Results According to surgical pathology or ERCP examination, 20 cases of lesions were located in the hilar and above segment, 42 cases of common bile duct, 27 cases of pancreatic segment, and 3 cases of ampullary segment. The accuracy of MRI combined with MRCP in the diagnosis of bile duct obstruction disease was higher than that of MRCP examination alone, with statistically significant difference(P<0.05);50 cases of benign lesions and 42 cases of malignant lesions were diagnosed by surgical pathology or ERCP examination. The accuracy of MRI combined with MRCP in the qualitative diagnosis of benign and malignant lesions was higher than that of MRCP examination alone, with statistically significant difference(P<0.05). Conclusion Compared with MRCP examination alone, MRI combined with MRCP examination has higher accuracy in localization and qualitative diagnosis of bile duct obstruction diseases.
