摘要
目的探讨磁共振成像(MRI)在不同分期的膝半月板及关节软骨损伤中的诊断价值。方法选取2018年2月至2020年1月于本院就诊的疑似膝半月板以及关节软骨损伤患者76例,对所有患者均行MRI检查,以膝关节镜结果作为金标准,分析MRI在不同分期的膝半月板及关节软骨损伤中的诊断价值。结果76例患者中经膝关节镜结果证实,存在膝半月板损伤69个,其中Ⅰ级15个,占比21.74%,Ⅱ级22个,占比31.88%,Ⅲ级32个,占比46.38%;存在关节软骨损伤61个,其中Ⅰ期12个,占比19.67%,Ⅱa期10个,占比16.39%,Ⅱb期11个,占比18.03%,Ⅲ期15个,占比24.59%,Ⅳ期13个,占比21.31%。以膝关节镜结果为金标准,MRI对不同分期的膝半月板损伤的诊断具有极好的一致性(Kappa=0.818);MRI对不同分期关节软骨损伤的诊断具有极好的一致性(Kappa=0.794)。结论MRI在不同分期的膝半月板及关节软骨损伤的诊断中具有较高应用价值,可为诊断与治疗该病提供重要依据,值得临床推广应用。
Objective To investigate the diagnostic value of magnetic resonance imaging(MRI)in the knee meniscus and articular cartilage injury at different stages.Methods 76 patients with suspected knee meniscus and articular cartilage injury who were treated in our hospital from February 2018 to January 2020 were selected.All patients were examined by MRI.The results of knee arthroscopy were used as the gold standard to analyze MRI.The diagnostic value in different stages of knee meniscus and articular cartilage injury.Results The knee arthroscopy confirmed that there were 69 knee meniscus injuries in the selected 76 patients,of which 15 were gradeⅠ,accounting for 21.74%,22 were gradeⅡ,accounting for 31.88%,and 32 were gradeⅢ,accounting for 46.38%;there are 61 articular cartilage injuries,including 12 in stageⅠ,accounting for 19.67%,10 in stageⅡa,accounting for 16.39%,11 in stageⅡb,accounting for 18.03%,and 15 in stageⅢ,accounting for 24.59%,13 phaseⅣ,accounting for 21.31%.Taking knee arthroscopy results as the gold standard,MRI has excellent consistency in the diagnosis of knee meniscus injury in different stages(Kappa=0.818);MRI has excellent consistency in the diagnosis of articular cartilage injury in different stages(Kappa=0.794).Conclusion MRI is of high application value in the diagnosis of knee meniscus and articular cartilage injuries at different stages,which can provide an important basis for the diagnosis and treatment of the disease,and it is worthy of clinical promotion and application.
