摘要
目的目前关于晚发型子痫前期孕产妇合并胎儿生长受限(FGR)的危险因素研究较少。文中旨在探究子痫前期孕产妇并发FGR的危险因素。方法回顾性分析2015年1月至2020年12月在南京医科大学附属苏州市立医院产科分娩的252例孕产妇临床资料。根据是否并发FGR分为两组:合并FGR组(晚发型子痫前期合并FGR的产妇,n=47)、无FGR组(晚发型子痫前期无合并FGR的产妇,n=205)。采用Loglstic回归分析子痫前期孕产妇并发FGR的危险因素。结果羊水过少(OR=7.743,95%CI:1.317-45.518),尿蛋白≥3+(OR=2.745,95%CI:1.270-5.929)、清蛋白(OR=0.915,95%CI:0.838~0.999),总胆红素(OR=0.871,95%CI:0.791〜0.959)是子痫前期并发FGR的危险因素、多因素回归模型预测晚发型子痫前期并发FGR的ROC曲线显示AUC为0.804,95%CI:0.739~0.868,表明模型的拟合度良好。结论晚发型子痫前期孕妇诊疗过程中应密切监测羊水过少、大量蛋白尿、低血清白蛋白、低血清胆红素的发生,做到早期预防FGR。
Objective There are few studies on the risk factors of late onset preeclampsia complicated with fetal growth restriction(FGR).The purpose of this study was to investigate the risk factors of FGR in preeclampsia pregnant women.Methods Clinical data of 252 pregnant women who delivered in Suzhou City Hospital affiliated to Nanjing Medical University from January 2015 to December 2020 were retrospectively analyzed.According to the complication of FGR,they were divided into two groups:group with FGR(late onset preeclampsia women with FGR,n=41)and group without FGR(late onset preeclampsia women without FGR,n=205).Logistic regression was used to analyze the risk factors of FGR in pregnant women with preeclampsia.Results Oligohydram-nios(OR=7.743,95%CI:1.317-45.518),urinary protein≥3+(OR=2.745,95%CI:1.270-5.929),albumin(OR=0.915,95%CI:0.838-0.999),Total bilirubin(OR=0.871,95%CI:0.791〜0.959)was a risk factor for FGR in preeclampsia.The ROC curve of the multi-factor regression model for predicting late-onset preeclampsia complicated with FGR showed an AUC of 0.804(95%CI:0.739-0.868),indicating a good fitting degree of the model.Conclusion The occurrence of oligohydramnios,macroalbuminuria,low serum albumin and low serum bilirubin should be closely monitored during the diagnosis and treatment of late onset preeclampsia,so as to prevent FGR at an early stage.
