摘要
目的:检索脑卒中后康复中有关言语功能障碍病人的护理与管理方面的相关证据,并对最佳证据进行总结。方法:检索中国知网、临床诊疗指南、JBI循证卫生保健中心数据库、加拿大安大略注册护理协会(RNAO)、英国国家临床医学研究所指南库(NICE)、美国指南网(NGC)、PubMed、Web of Science、EMbase、BMJ Best Practice、the Cochrane Library、Up to Date中关于脑卒中后言语功能障碍病人的护理与管理的证据,包括指南、专家共识、证据总结、系统评价、随机对照试验、类试验研究及队列研究。由2名循证护理方向的研究生分别对纳入的文献进行质量评价,并对达到纳入及排除标准的研究进行证据提取及总结。检索时限为2016年1月1日—2020年11月22日。结果:最终纳入14篇文献,其中指南4篇,随机对照试验10篇。总结了9条最佳证据。结论:现有证据表明,在对脑卒中后言语功能障碍病人进行护理与管理时,要注重对病人个性化的评估,制定适合病人的康复方案,以合理有效的方式帮助病人康复,提高病人的生活质量,帮助病人社交。
Objective:To search and summarize the best evidence on nursing and management of speech dysfunction in post-stroke rehabilitation.Methods:Evidence on the nursing and management of patients with speech dysfunction after stroke was retrieved from CNKI,Clinical Guidelines,Joanna Briggs Institute,RNAO,NICE,NGC,PubMed,Web of Science,EMbase,BMJ Best Practice,the Cochrane Library,Up to Date.Evidence includes guidelines,expert consensus,evidence summaries,systematic reviews,randomized controlled trials,quasi-experimental study,and cohort studies.The quality of the included literatures was evaluated by 2 graduate students majoring in evidence-based nursing,and evidence extraction and summary were conducted for the studies that met the inclusion and exclusion criteria.The retrieval time was from January 1,2016 to November 22,2020.Results:A total of 14 articles were included,including 4 guidelines and 10 randomized controlled trials.The 9 best pieces of evidence were summarized.Conclusion:Existing evidence shows that in the nursing and management of patients with speech dysfunction after stroke,it is necessary to pay attention to the personalized assessment of patients,formulate rehabilitation programs suitable for patients,help patients recover in a reasonable and effective way,improve the quality of life of patients,and help patients socialize.
