摘要
食管癌是一种常见的消化道恶性肿瘤,我国的食管癌患者数量庞大,并且病理类型多为鳞状细胞癌,患者的5年生存率并不理想。手术切除是食管癌中早期治疗最重要且不可替代的治疗手段,病变切除后还需要进行合理的消化道重建。传统的全胃代食管手术在维持消化道功能和减轻手术并发症方面效果比较有限,因此优势较为明显的管状胃成形术在食管癌切除后的消化道重建中逐渐占据主导地位。但由于吻合口问题、胃排空延迟等并发症仍时有出现,并且一旦发生就会对患者造成严重不良影响,因此仍有大量研究着眼于对其进行改良和革新,而且在荧光成像、血管解剖等领域的新发现也为管状胃的改良提供了新的思路。文章对近年来食管癌切除后管状胃成型及其改良术式的研究情况及其应用前景进行综述。
Esophageal cancer is a common malignant tumor of the digestive system.There are a huge number of patients with esophageal cancer in China,and the pathological types are mostly squamous cell carcinoma.The 5-year survival rate of the patients is still not satisfactory.Surgical resection is the most important and irreplaceable method for the early treatment of esophageal cancer.Nevertheless,reasonable digestive tract reconstruction is also needed after lesion resection.The traditional reconstruction by replacing the esophagus using the whole stomach has a limited effect in maintaining the function of digestive tract and reducing the complications.Therefore,tubular gastroplasty is becoming increasingly dominant in the reconstruction of digestive tract after resection of esophageal cancer.However,complications such as anastomotic problems and delayed gastric emptying can cause serious adverse effects.There are still a lot of studies focusing on the improvement and innovation of tubular gastroplasty.Furthermore,new discoveries in fluorescence imaging,vascular anatomy and other fields also provide new ideas for the improvement of tubular stomach.This paper reviews the recent research progress and application prospect of tubular gastroplasty and its modified surgery after resection of esophageal carcinoma.
