摘要
目的2型糖尿病合并动脉硬化性心血管疾病(ASCVD)患者应用卡格列净后观察血清中鸢尾素浓度变化,研究卡格列净治疗2型糖尿病(T2DM)合并ASCVD的相关机制。方法入选锦州医科大学附属第一医院诊疗的2型糖尿病患者144例,根据ASCVD诊断标准分为单纯T2DM组(A组)84例和T2DM合并ASCVD组(B组)60例。两组患者均给予常规降糖药和卡格列净降糖治疗,即单纯T2DM患者42例给予常规降糖药物(A1组),42例口服常规降糖药联用卡格列净(A2组);合并ASCVD患者30例给予常规降糖药(B1组),口服常规降糖药联用卡格列净30例(B2组)。T2DM合并ASCVD组患者颈动脉中膜厚度均大于1.0mm。四组患者用药3个月后,分别检测血清中鸢尾素(Irisin)浓度、颈动脉中膜厚度(IMT)、左室射血分数(LVEF)及血糖、血脂等,探讨其相关机制。结果单纯T2DM组和T2DM合并ASCVD组中,应用卡格列净组BMI、SBP、FPG、HOMA-IR、IMT、HbAlc、LVEF、TC、TG均低于传统降糖组;在相同用药条件下,T2DM合并ASCVD组均高于单纯T2DM。在单纯T2DM组和T2DM合并ASCVD组中,应用卡格列净鸢尾素高于传统降糖药物治疗;相同用药条件下,T2DM合并ASCVD组较单纯T2DM组鸢尾素水平降明显降低,差异有统计学意义(P<0.05)。鸢尾素与BMI、SBP、FPG、HOMA-IR、IMT、HbAlc、LVEF、TC、TG呈负相关(P<0.05)。结论卡格列净治疗ASCVD可能与鸢尾素密切相关。
Objective To observe the changes of serum irisin concentration in patients with type 2 diabetes mellitus complicated with cardiovascular disease(ASCVD)after treatment with kagligin,and to explore the related mechanism of kagligin in the treatment of type 2 diabetes mellitus(T2DM)complicated with ASCVD.Methods A total of 144 T2DM patients were enrolled in the First Affiliated Hospital of Jinzhou Medical University.According to the diagnostic criteria of ASCVD,they were divided into two groups:84 T2DM patients(group A)and 60 T2DM patients with ASCVD(group B).Patients in both groups were treated with conventional hypoglycemic drugs and cagliazin,namely,42 T2DM patients were treated with conventional hypoglycemic drugs(group A1),and 42 patients were treated with conventional oral hypoglycemic drugs combined with cagliazin(group A2).30 patients with ASCVD were given conventional hypoglycemic drugs(group B1),conventional oral hypoglycemic drugs combined with cagliazin in 30 patients(group B2).Carotid media thickness of T2DM patients with ASCVD was greater than 1.0mm.After 3 months of medication,serum irisin concentration,carotid artery media thickness(IMT),left ventricular ejection fraction(LVEF),blood glucose,blood lipid,etc.,were detected in the four groups,to explore the related mechanism.Results BMI,SBP,FPG,HOMA-IR,IMT,HbAlc,LVEF,TC and TG in T2DM alone and T2DM combined with ASCVD group were lower than those in traditional hypoglycemic group.Under the same medication condition,T2DM combined with ASCVD group was higher than T2DM alone.In T2DM group alone and T2DM group combined with ASCVD,the application of cagliazin Irisin was higher than that of traditional hypoglycemic drugs.Under the same medication conditions,the level of Irisin in T2DM combined with ASCVD group was significantly lower than that in T2DM alone.The difference was statistically significant(P<0.05).Irisin was negatively correlated with BMI,SBP,FPG,HOMA-IR,IMT,HbAlc,LVEF,TC and TG(P<0.05).Conclusion Cagliazin may be closely related to Irisin in the
