iASSIST智能导航与传统技术在TKA中的早期疗效比较

摘要 目的:比较iASSIST智能辅助导航系统与传统技术在全膝关节置换术中的早期疗效.方法:回顾性分析2017年9月-2019年9月南京市第一医院运动关节科行初次全膝关节置换的原发性膝关节骨关节炎患者60例(年龄≥70岁).患者随机分2组,各30例分别接受传统手术技术和Zimmer-iASSIST智能辅助导航系统行全膝关节置换术.术后随访临床和影像学资料,分析比较围术期的相关指标、影像学资料及术后功能.结果:2组患者年龄、BMI、性别、患肢左/右侧对比无显著性差异(P>0.05).导航组手术时间比对照组明显缩短11分钟(P<0.01).导航组失血量及输血人数均优于对照组(P<0.05).股骨冠状角、胫骨冠状角、股骨矢状角和胫骨矢状角在术后第1周内复查中无明显差异,但是对照组术后1年出现多个角度明显丢失(P<0.05).术后1年随访,2组患者临床和功能均恢复大致正常,但导航组的膝关节活动度、KSS评分、HSS评分和VAS评分均优于对照组.结论:iASSIST导航系统是安全可靠的精准定位假体位置的有效工具,且使用iASSIST导航系统的TKA较传统方法手术时间更短,治疗效果更好. Objective:To compare the early efficacy of the intelligent iASSIST-assisted navigation and traditional technology in TKA.Methods:60 patients(^70 years)with primary knee osteoarthritis who underwent primary TKA(2017.09-2019.09)in the joint and sports medicine department of Nanjing First Hospital were retrospectively analyzed.Patients were divided into two groups randomly(30 ca-ses in each group).Traditional surgical techniques and Zimmer iASSIST-assisted navigation were used respectively.The complete clinical and imaging data were obtained to compare operation related data,imaging data and postoperative function.Results:There was no significant difference in age,BMI,gender and left/right limb between the two groups(P>0.05).The operation time in the navigation group was significantly shorter than that in the control group by 11 minutes(P<0.01).The blood loss and the number of blcx>d transfusion in the navigation group were better than those in the control group(P<0.05).There was no significant difference in coronal femoral component angle,coronal tibia component angle,sagittal femoral component angle and sagittal tibia component angle within the first week after operation,but multiple angles were significantly lost in the control group one year after operation(P<0.05).In 1 year followup,the clinical and function of the two groups returned to normal,but the knee range of motion,KSS score,HSS score and VAS score of the navigation group were better than those of the control group.Conclusion:iASSIST-assisted navigation is a safe and effective tool to determine the position of component.Moreover,TKA using iASSIST-assisted navigation has shorter operation time and better postoperative effect than traditional methods.
