摘要
目的利用伽马—泊松收缩估计法(Empirical Bayes Gamma-Possion Shrinker,GPS)检测2017—2019年甘肃省预防接种异常反应可疑信号。方法采用GPS法和分层GPS法对2017—2019年甘肃省报告的疑似预防接种异常反应中的信号进行检测。结果一般反应中,性别和年龄分层后百白破b型流感嗜血杆菌(DTap-Hib)联合疫苗—硬结组合仍然是强信号,DTap-Hib联合疫苗—红肿组合、白破疫苗(儿童)—硬结组合是可疑信号。非一般反应未发现强信号,在性别分层后卡介苗—卡介苗淋巴结炎组合、A+C群流脑疫苗(多糖)—过敏性紫癜组合、卡介苗—局部脓肿组合、乙脑疫苗(减毒)—荨麻疹组合是可疑信号。结论需要采用更严格的方法来评估GPS检测到百白破b型流感嗜血杆菌(DTap-Hib)联合疫苗—硬结组合等信号的因果关系,对确认存在因果关系的信号应改进疫苗生产工艺。
Objective Using the Gamma-Poisson Shrinkage Estimation(GPS) to detect the paradoxical signal of vaccine reactions in Gansu Province from 2017 to 2019.Methods GPS detection and stratification GPS method were used to detect suspicious reaction signals of vaccine reactions in Gansu Province from 2017 to 2019.Results In common vaccine reaction,the Diphtheria Tetanus Pertussis-Hib combination vaccine-induration combination was still a strong signal after gender and age stratification.Diphtheria Tetanus Pertussis-Haemophilus influenzae Type b(DTap-Hib)vaccine-redness and swelling combination,Tetanus and diphtheria toxoid children’s dose vaccine-induration were suspicious signals.No strong signal was found.After gender stratification,Bacillus Calmette Guérin(BCG)-BCG lymphadenitis combination-Meningococcal A+C vaccine-Henoch-Schnlein Purpura,BCG-local abscess and live attenuated Encephalitis vaccines-Urticaria are paradoxical signals.Conclusion DTap-Hib vaccine-induration and other signals from GPS detected require a more rigorous method to evaluate its causality,and the vaccine production process should be improved for confirmed causality.
