摘要
目的分析老年慢性阻塞性肺疾病(COPD)患者血清α-HBD水平与其病情严重程度的相关性。方法选择2017年1月至2019年12月我院收治的120例COPD患者作为研究对象,采用一秒用力呼气容积占预计值的百分比(FEV_(1)%pred)评估患者的病情严重程度并分为轻度组、中度组和重度组,检测所有患者的血清α-HBD水平并分析血清α-HBD水平与病情严重程度的相关性。结果120例老年COPD患者中,轻度57例,中度43例,重度20例;重度组的FEV_(1)%pred低于轻度组、中度组,且中度组的FEV_(1)%pred低于轻度组,差异有统计学意义(P<0.05);重度组的血清α-HBD水平均高于轻度组、中度组,且中度组的血清α-HBD水平高于轻度组,差异有统计学意义(P<0.05)。双变量Pearson直线相关性分析显示,老年COPD患者的血清α-HBD水平与FEV_(1)%pred呈显著负相关(r=-0.876,P=0.000)。Logistic回归分析显示,血清α-HBD水平是COPD患者病情严重程度的影响因素(OR>1,P<0.05)。结论老年COPD患者的血清α-HBD水平与其病情严重程度密切相关,临床可通过早期监测患者的血清α-HBD水平,及时掌握患者病情,并采取有效干预措施,以有效延缓其病情进展。
Objective To analyze the correlation between serum α-HBD level and severity of the disease of elderly patients with chronic obstructive pulmonary disease(COPD).Methods A total of 120 patients with COPD admitted to our hospital from January 2017 to December 2019 were selected as research objects.The percentage of forced expiratory volume in one second in the predicted value(FEV_(1)%pred)was used to evaluate the severity of the disease,and all cases were divided into mild group,moderate group and severe group.The serum α-HBD levels of all patients were detected and the correlation between serum α-HBD level and the severity of the disease was analyzed.Results Among 120 cases of elderly patients with COPD,57 cases were mild,43 cases were moderate,20 cases were severe.The FEV_(1)%pred of the severe group was lower than that of the mild group and the moderate group,and the FEV_(1)%pred of the moderate group was lower than that of the mild group(P<0.05).The serum α-HBD level of the severe group was higher than that of the mild group and the moderate group,and the serum α-HBD level of the moderate group was higher than that of the mild group(P<0.05).The bivariate Pearson linear correlation analysis showed that the serum α-HBD level in elderly patients with COPD was significantly negatively correlated with FEV_(1)%pred(r=-0.876,P=0.000).Logistic regression analysis showed that serum α-HBD level was an influencing factor of severity of the disease of COPD patients(OR>1,P<0.05).Conclusions The level of serum α-HBD in elderly patients with COPD is closely related to the severity of the disease.Early monitoring of serum α-HBD levels of patients can timely grasp patients'disease condition and take effective intervention measures to effectively delay the progression of the disease.
