摘要
目的探讨血清25羟维生素D25(OH)D水平与肺腺癌合并2型糖尿病的相关性。方法回顾性分析郑州大学第一附属医院2018年8月至2020年3月新确诊的肺腺癌合并2型糖尿病患者83例,选取同期我院就诊的单纯2型糖尿病患者65例和健康对照者80例,比较3组间血清25(OH)D水平,并分析肺腺癌合并糖尿病组血清25(OH)D水平与临床病理特征的关系。结果肺腺癌合并糖尿病组血清25(OH)D水平(16.28±3.30)μg/L明显低于单纯糖尿病组(23.67±5.97)μg/L和健康对照组(29.74±6.37)μg/L(F=130.458,P<0.001)。肺腺癌合并糖尿病患者中,血清25(OH)D水平与腺癌分化程度(F=23.921,P<0.001)、肿瘤大小(t=6.948,P<0.001)、淋巴结转移(t=3.823,P<0.001)、远端转移(t=4.701,P<0.001)及糖化血红蛋白水平(t=4.854,P<0.001)关系密切。结论血清25(OH)D与肺腺癌和糖尿病的发生密切相关,且与肿瘤的大小、分化程度以及有无转移相关。
Objective To investigate the correlation between serum 25 hydroxyvitamin D[25(OH)D]level and lung adenocarcinoma in patients with type 2 diabetes.Methods A retrospective analysis of 83 newly diagnosed lung adenocarcinoma patients with type 2 diabetes from August 2018 to March 2020 in the First Affiliated Hospital of Zhengzhou University was conducted.Sixty-five patients with simple type 2 diabetes and 80 healthy controls in our hospital during the same period were selected.The serum 25(OH)D levels in the three groups were compared.The relationship between serum 25(OH)D level and clinicopathological characteristics in patients with lung adenocarcinoma and diabetes mellitus was analyzed.Results The serum 25(OH)D levels in the lung adenocarcinoma combined with diabetes group[(16.28±3.30)μg/L]were significantly lower than those in the simple diabetes group[(23.67±5.97)μg/L]and the healthy control group[(29.74±6.37)μg/L],(F=130.458,P<0.001).In the patients with lung adenocarcinoma and diabetes,serum 25(OH)D levels were closely related to the degree of differentiation(F=23.921,P<0.001),tumor size(t=6.948,P<0.001)、lymph node metastasis(t=3.823,P<0.001)、distal metastasis(t=4.701,P<0.001)and glycosylated hemoglobin(t=4.854,P<0.001).Conclusion Serum 25(OH)D is closely related to the occurrence of lung adenocarcinoma and diabetes,and is related to tumor size,differentiation and metastasis.
