摘要
目的探讨特制气管插管在气管插管全麻手术中的应用效果。方法将140例气管插管全麻手术患者按随机数字表法分为两组,每组70例。对照组采用常规气管插管,表面涂抹利多卡因乳膏进行气管插管表面麻醉;观察组采用特制气管插管,注药囊内给予利多卡因麻醉。比较两组麻醉效果、不良反应发生率,拔管即刻及拔管后1 min、3 min、5 min血压、心率变化,麻醉起效时间、手术时间、术后苏醒时间、麻醉维持用药量,拔管后5 min、10 min、30 min咽部疼痛情况。结果观察组麻醉优良率略高于对照组,但组间比较差异无统计学意义(P>0.05)。观察组拔管即刻及拔管后1 min、3 min、5 min血压、心率显著优于对照组(P<0.01),麻醉起效时间、术后苏醒时间均显著短于对照组(P<0.01),麻醉维持用药丙泊酚和瑞芬太尼用量均显著少于对照组(P<0.01),两组手术时间比较差异无统计学意义(P>0.05)。观察组拔管后5 min、10 min、30 min咽部视觉模拟评分法评分显著低于对照组(P<0.01),不良反应发生率显著低于对照组(P<0.05)。结论特制气管插管在气管插管全麻手术中的应用效果满意,能在保证麻醉优良率及镇痛效果的同时,减轻气管插管对呼吸道的刺激,减少麻醉维持用药量,缓解咽部疼痛,安全性高。
Objective To investigate the application effect of special endotracheal intubation in general anesthesia for endotracheal intubation.Methods A total of 140 patients with general anesthesia for endotracheal intubation were divided into two groups according to random number table method,with 70 cases in each group.The control group was treated with conventional endotracheal intubation and coated with lidocaine cream for endotracheal intubation anesthesia.On this basis,the observation group was treated with special endotracheal intubation and lidocaine in the drug capsule.The anesthetic effect,incidence of adverse reactions,changes of blood pressure and heart rate immediately after extubation and 1 min,3 min and 5 min after extubation,anesthesia onset time,operation time,postoperative recovery time,dosage of anesthesia maintenance,pharyngeal pain 5min,10min and 30min after extubation were compared between the two groups.Results The excellent rate of anesthesia in observation group was slightly higher than that in control group,but there was no significant difference between the groups(P>0.05).The blood pressure and heart rate in observation group immediately after extubation and 1 min,3 min and 5 min after extubation were significantly better than those in control group(P<0.01).The anesthesia onset time and postoperative recovery time were significantly shorter than those in control group(P<0.01),and the dosage of propofol and remifentanil were significantly less than those in control group(P<0.01).There was no significant difference in operation time between the two groups(P>0.05).Conclusions The application effect of special endotracheal intubation in general anesthesia for endotracheal intubation is satisfactory.It can not only ensure the excellent rate of anesthesia and analgesic effect,but also reduce the stimulation of endotracheal intubation to respiratory tract,reduce the dosage of anesthesia maintenance,alleviate pharyngeal pain,and has high safety.
