摘要
目的评估孕前献血对产妇分娩前铁营养状况及血红蛋白(Hb)的影响。方法选取2019年11月1日—2020年11月30日本院和宁波市妇女儿童医院待产、单胎且足月(妊娠≥37周)妊娠的产妇14671例,收集其基本临床信息和相关实验指标,并通过产妇支付宝获取其浙江省内详细的献全血记录。结果孕前3年内及孕前累计在浙江省内献全血率分别为4.57%和14.53%。血清铁蛋白(SF)、血清铁(SI)及Hb在孕前3年内及孕前累计各献血组间比较发现低频献血组高于非献血组(P<0.05),而转铁结合力(TIBC)只在孕前累计各献血组间比较发现非献血组高于低频献血组(P<0.05)。产妇总的SF缺乏率为7.2%,贫血率为9.7%。SF缺乏率在各献血组比较,未献血组高于低频献血组(P<0.05)。贫血率在孕前累计献血各组间未献血组高于低频及高频献血组,上述差异均有统计学意义(P<0.05)。经多元线性回归校正分析后发现,孕前累计献血次数与SF及Hb正相关(P<0.05)。结论在妊娠期可能存在补充铁剂、叶酸及维生素B12等干预措施下,孕前适度献血并不会造成产妇分娩前SF缺乏及贫血增加。
Objective To evaluate the effect of blood donation before pregnancy on iron nutrition and hemoglobin(Hb)before delivery.Methods A total of 14671 pregnant women who were waiting for delivery,singleton,and full term(gestational≥37 weeks)in our hospital and Ningbo Women's and Children's Hospital were enrolled from January November 1,2019 to November 30,2020.The basic clinical information and related experimental indexes were collected,and the detailed blood donation records in Zhejiang Province before pregnancy were obtained through Alipay.Results The whole blood donation rate in Zhejiang Province within three years before and accumulated before pregnancy was respectively 4.57%and 14.53%.Serum ferritin(SF),serum iron(SI)and Hb were higher in low-frequency blood donation group than that in non-blood donation group(P<0.05),while total iron binding capacity(TIBC)was only higher in non-blood donation group than that in low-frequency blood donation group(P<0.05).The total SF deficiency rate was 7.2%and anemia rate was 9.7%.The SF deficiency rate in the non-donation group was higher than that in the low frequency donation group(P<0.05).The anemia rate of the non-donation group was higher than that of the low frequency and high frequency blood donation groups(P<0.05).After multiple linear regression analysis,the cumulative number of blood donation before pregnancy was positively correlated with SF and Hb(P<0.05).Conclusion With the possible supplement of iron,folic acid and vitamin B12 during pregnancy,moderate blood donation before pregnancy will not cause maternal SF deficiency and anemia increase before delivery.
