摘要
目的分析血清同型半胱氨酸(Hcy)、尿酸(UA)水平与老年帕金森病(PD)患者蒙特利尔认知评估量表(MoCA)评分的相关性。方法选取2019年1月至2021年3月河南大学淮河医院收治的115例PD患者及97例同期健康体检者为研究对象,将PD患者纳入PD组,健康体检者纳入健康组。对比两组血清Hcy、UA水平及MoCA评分,比较PD组不同认知情况血清Hcy、UA水平,并分析PD患者血清Hcy、UA水平与MoCA评分的相关性。结果PD组Hcy水平高于健康组,UA水平低于健康组(P<0.05);PD组MoCA视空间与执行能力、语言、注意、抽象思维、命名、定向力、延迟记忆评分均低于健康组(P<0.05);不同认知障碍PD患者血清Hcy、UA水平比较,差异有统计学意义(P<0.05);PD患者血清Hcy水平与MoCA评分呈负相关,血清UA水平与MoCA评分呈正相关(P<0.05);受试者工作特征(receiver operating characteristic,ROC)曲线分析显示,血清Hcy、UA水平预测PD患者中重度认知障碍的敏感度(76.71%、84.93%)、特异度(80.95%、78.57%)均较高,且联合检测敏感度(93.15%)、特异度(90.48%)更高(P<0.05)。结论血清Hcy、UA水平可预测PD患者MoCA评分,血清MoCA评分与Hcy水平呈负相关,与UA水平呈正相关,检测血清Hcy、UA水平可评估患者认知功能及预后,对临床治疗具有指导作用。
Objective To analyze the correlation between serum homocysteine(Hcy)and uric acid(UA)levels and Montreal cognitive assessment(MoCA)scores in elderly patients with Parkinson’s disease(PD).Methods A total of 115 cases of PD patients and 97 cases of healthy physical examination in Huaihe Hospital of Henan University from January 2019 to March 2021 were selected as the research objects.The PD patients were divided into the PD group and the healthy subjects were divided into the healthy group.The serum Hcy and UA levels of the two groups were compared.The MoCA score,serum Hcy and UA levels of different cognitive conditions in the PD group were compared,and the correlation between the serum Hcy and UA levels of the PD patients and the MoCA score was analyzed.Results The level of Hcy in the PD group was higher than that in the healthy group,and the level of UA was lower than that in the healthy group(P<0.05).The PD group’s MoCA visual space and executive ability,language,attention,abstract thinking,naming,orientation and delayed memory scores were all lower than those in the healthy group(P<0.05).There was statistically significant difference in the levels of serum Hcy and UA in PD patients with different cognitive impairment(P<0.05).The level of serum Hcy in PD patients was negatively correlated with the MoCA score,and the level of serum UA was positively correlated with the MoCA score(P<0.05).ROC curve analysis showed that the sensitivity(76.71%,84.93%)and specificity(80.95%,78.57%)of serum Hcy and UA levels in predicting moderate and severe cognitive impairment in PD patients were high,and the combined detection sensitivity(93.15%)and the specificity(90.48%)were higher.Conclusion Serum Hcy and UA levels can predict the MoCA scores of PD patients.Serum MoCA scores are inversely proportional to Hcy levels and directly proportional to UA levels.The detection of serum Hcy and UA levels can assess the patient’s cognitive function and prognosis,and has a guiding role in clinical treatment.
