摘要
目的探讨强直性脊柱炎(AS)脊柱的磁共振成像(MRI)特征。方法选取2017年1月至2019年12月濮阳市安阳地区医院收治的38例接受脊柱MRI检查的AS患者为研究对象,分析脊柱影像学特点及脊柱附属结构改变。结果38例AS患者中,MRI扫描结果显示MR角征32例(84%),滑膜关节炎8例(21%),椎间盘炎6例(16%),附着点炎2例(5%),MR角征发生率最高(P<0.009)。MR角征累及下胸椎单元和腰椎单元多于颈椎单元(P<0.05)。MR角征累及椎体前角多于后角(P<0.009),信号改变以ModicⅡ型多见(89%)。MR角征累及3个及以上脊椎单元多见(94%)。仅表现为MR角征者19例(59%),MR角征与其他炎性病变并存者13例(41%)。结论MR角征是AS累及脊柱时最常见表现,病变以胸腰椎单元多见,呈多椎体角分布,可单独或与其他炎性病变混合存在。脊柱MRI检查出现MR角征,应该考虑强直性脊柱炎的可能。
Objective To investigate the magnetic resonance imaging(MRI)characteristics of the spine in ankylosing spondylitis(AS).Methods Thirty-eight patients with AS in the Anyang District Hospital of Puyang City from January 2017 to December 2019 were selected as the research subjects,and all patients were examined by spine MRI.The MRI characteristics of the spine and the change of spinal accessory structure were analyzed.Results Among the 38 patients with AS,MRI scan results showed 32 cases(84%)of MR angle sign,8 cases(21%)of synovial arthritis,6 cases(16%)of intervertebral discitis,and 2 cases(5%)of enthesitis.The incidence of MR angle sign was the highest(P<0.009).The MR angle sign involved more lower thoracic unit and lumbar unit than cervical unit(P<0.009).The MR angle sign involved more anterior horns than the posterior horns(P<0.05),and the most frequent change of signal intensity was Modic typeⅡ(89%).The MR angle sign with three or more spine units was mostly seen(94%).There were 19 cases(59%)of mere MR angle sign,and 13 cases(41%)of the MR angle sign with other inflammatory lesions.Conclusion The MR angle sign is the most common manifestation when AS involves the spine.The lesions are more common in thoracolumbar units,with multiple vertebral body angles,which can exist alone or mixed with other inflammatory lesions.MRI of the spine reveals the MR angle sign,and the possibility of ankylosing spondylitis should be considered.
