摘要
目的:分析名老中医治疗胃食管反流病(GERD)的用药规律。方法:检索中国知网数据库(CNKI)、维普中文期刊服务平台(VIP)、万方数据知识服务平台(WanFang Data)等中文数据以及手工检索《国医大师验案良方》及《中国百年百名中医临床家》系列丛书中记载的名老中医治疗GERD的医案,按照纳入和排除标准对所收集的医案进行筛选。结果:最终纳入18名国家级名老中医的32则GERD医案,共计61诊次61方。对纳入处方整理并建立数据信息库,运用频数分析法、聚类分析法、关联分析法进行数据挖掘。频数分析得到高频药物47味,用药频次排在前10的中药依次是半夏、海螵蛸、陈皮、枳壳、白芍、蒲公英、黄连、茯苓、贝母、甘草,用药类别以理气药、补益药、化痰止咳平喘药为主,其次为化湿药、消食药、利水渗湿药。聚类分析得到12组聚类药组,体现出疏肝理气、健脾化湿、制酸止痛、消食导滞的治法特色。关联分析得到常用药对关联规则10条,3味药药对关联规则8条,4味药药对关联规则11条。结论:体现出名老中医治疗GERD的核心用药为化痰止咳平喘药、利水渗湿药、收敛固涩药之间的配伍使用。数据挖掘名老中医治疗GERD的用药规律,望为临床探索治疗GERD提供启示。
Objective:To analyze the medication rules of famous and old Chinese medicine practitioners in the treatment of gastroesophageal reflux disease (GERD).Methods:Retrieving Chinese data such as CNKI,VIP Chinese Journal Service Platform (VIP),WanFang Data Knowledge Service Platform (WanFang Data) and other Chinese data,as well as manual retrieval of “Chinese Medicine Master’s Prescriptions and Remedies” and “One Hundred Years of Traditional Chinese Medicine in China” The medical records of famous old Chinese doctors in the treatment of GERD recorded in the “Clinicians” series are screened according to the inclusion and exclusion criteria.Results:In the end,32 GERD medical records of 18 national-level famous TCM doctors were included,with a total of 61 visits and 61 parties.Arrange the included prescriptions and establish a data information database,and use frequency analysis,cluster analysis,and association analysis to conduct data mining.Frequency analysis revealed 47 high-frequency medicines.The top 10 Chinese medicines were Pinellia,Sea Cucumber,Tangerine Peel,Citrus aurantium,Radix Paeoniae Rubra,Dandelion,Coptis,Poria,Fritillaria,and Licorice.,Tonic medicine,phlegm-relieving cough and asthma medicine are the main medicines,followed by dampness medicine,antiseptic medicine,diuresis and dampness medicine.Cluster analysis resulted in 12 cluster drug groups,which reflected the characteristics of the treatment methods of soothing the liver and regulating qi,invigorating the spleen and dissipating dampness,relieving soreness and pain,and eliminating food and promoting stagnation.The association analysis obtained 10 common drug-to-medicine association rules,8 three-medicine-to-medicine association rules,and 11 four-medicine-to-medicine association rules.Conclusion:It shows that the core medicines used by famous traditional Chinese medicine doctors to treat GERD are the compatibility of medicines for reducing phlegm and relieving cough and asthma,diuresis and dampness medicine,and astringent and astringent
