摘要
目的:比较舒芬太尼、瑞芬太尼在急诊脑外伤手术麻醉中的应用效果。方法:选取2017年1月至2018年12月解放军联勤保障部队第990医院急诊科收治的脑外伤患者80例,按照随机数字表法,分为对照组和观察组,各40例。对照组应用瑞芬太尼麻醉,观察组应用舒芬太尼麻醉,比较两组患者麻醉诱导前(T0)、插管时(T1)、手术开始时(T2)、拔管时(T3)的平均动脉压(MAP)和心率(HR)水平,同时对两组患者麻醉维持时间、气管插管拔管时间、苏醒时间以及不良反应发生率进行比较。结果:观察组患者T1、T2、T3的MAP、HR水平均较对照组平稳,差异具有统计学意义(P<0.05);观察组患者麻醉维持时间长于对照组,气管插管拔管时间、苏醒时间短于对照组,差异具有统计学意义(P<0.05);观察组患者不良反应发生率低于对照组,差异具有统计学意义(P<0.05)。结论:急诊脑外伤手术麻醉中舒芬太尼的应用效果优于瑞芬太尼,不良反应少,安全性高。
Objective To compare the effect of Sufentanil and Remifentanil in emergency operation of traumatic brain injury.Method From January 2017 to December 2018,80 patients with traumatic brain injury admitted to the emergency department of the 990th Hospital of PLA Joint Logistics Support Force were divided into control group(40 cases)and observation group(40 cases).The mean arterial pressure(MAP)and heart rate(HR)before anesthesia induction(T0),during intubation(T1),at the beginning of operation(T2)and during extubation(T3)were compared between the control group and the observation group,the duration of anesthesia,extubation time,recovery time and the incidence of adverse reactions were compared between the two groups.Results The MAP and HR levels of T1,T2 and T3 in the observation group were significantly higher than those in the control group(P<0.05),the duration of anesthesia in the observation group was longer than that in the control group,and the time of extubation and recovery in the observation group was shorter than that in the control group,the differences were all statistically significant(P<0.05),and the incidence of adverse reactions in the observation group was lower than that in the control group(P<0.05).Conclusion Sufentanil is more effective than Remifentanil in emergency operation of brain trauma with less side effects and higher safety.
