新生儿呼吸窘迫综合征200例床旁数字化X线摄影分析 被引量:3

摘要 目的分析数字化X线摄影(DR)对新生儿呼吸窘迫综合征(NRDS)的诊断价值。方法搜集2017年2月至2019年2月来我院诊治并经临床证实的NRDS患儿200例,均有完整影像资料,并进行回顾性分析。结果本组患儿均在积极治疗中复查,观察病变演变过程及治疗效果。影像征象:透过度减低呈颗粒状改变188例占94.0%,支气管气相152例占76.0%,心膈缘模糊82例占41.0%,白肺12例占6.0%。并发症:动脉导管开放10例占5.0%,肺气漏30例占15.0%,肺出血15例占7.5%,支气管肺发育不良13例占6.5%,早产儿代谢性骨病2例占1.0%,坏死性小肠结肠炎3例占1.5%,200例中13例出现慢性肺损害,1例因肺出血死亡,其余均能自主呼吸出院。结论DR对NRDS的诊断、病变进展、治疗效果评价、预后评估等具有重要作用,新生儿对X线辐射较为敏感,因此DR是临床不可缺少的诊断依据。 Objective To analyze the diagnostic value of DR in neonatal respiratory distress syndrome.Methods A total of 200 children with neonatal respiratory distress syndrome who were diagnosed and clinically confirmed in our hospital from February 2017 to February 2019 were enrolled.All patients underwent multiple bedside DR examinations and had complete image data and were analyzed retrospectively.Results Chest radiographs of all patients were reviewed several times during active treatment.As for the imaging signs,there were granular changes in 188 cases(94.0%),bronchial gas phase in 152 cases(76.0%),cardiac and diaphragmatic margin in 82 cases(41.0%),white lung in 12 cases(6.0%),arterial catheter opening in 10 cases(5.0%),pulmonary air leakage in 30 cases(15.0%),pulmonary hemorrhage in 15 cases(7.5%),bronchopulmonary dysplasia in 13 cases(6.5%).Metabolic osteopathy was found in 2 premature infants(1.0%).Necrotizing enterocolitis was found in 3 cases(1.5%).Of 200 patients,13 had chronic lung damage,only one died of pulmonary hemorrhage,and the rest could breathe autonomously.Conclusions DR plays an important role in the diagnosis,pathological progress,therapeutic effect evaluation and prognosis evaluation of neonatal respiratory distress syndrome.Newborns are more sensitive to X-ray radiation,so DR is an indispensable clinical diagnostic basis.
