摘要
目的探讨急诊科对急性心力衰竭采用硝普钠联合硝酸甘油注射液治疗的临床效果及安全性。方法选取2020年1—12月我院收治的急性心力衰竭患者200例患者作为本次研究对象,将单纯应用硝普钠注射液治疗的患者纳入对照组,共100例;将应用硝普钠联合硝酸甘油治疗的100例患者纳入试验组。根据临床症状改善情况结合临床验证结果评估治疗效果,观察血清生化检验指标变化情况,记录不良反应的发生率。结果试验组治疗总有效率显著高于对照组(88.00%vs.66.00%),差异有统计学意义(P<0.05);试验组血清H-FABP、sST2、hs-CRP指标改善程度较对照组明显,差异有统计学意义(P<0.05);试验组心力衰竭症状改善耗时较对照组短[(25.80±2.10)min vs(.43.60±2.50)min],差异有统计学意义(P<0.05);试验组不良反应发生率较对照组低(7.00%vs.15.00%),差异有统计学意义(P<0.05)。结论相比于单纯应用硝普钠,采用硝普钠联合硝酸甘油用药对急性心力衰竭患者具有见效快、保护心肌细胞、促进心功能恢复、安全性高等优势。
Objective To explore the clinical effect and safety of the combined treatment of sodium nitroprusside and nitroglycerin injection in the emergency department for acute heart failure.Methods From January to December 2020,200 patients with acute heart failure admitted to our hospital were randomly selected as the subjects of this study,and patients who were treated with sodium nitroprusside injection alone were included in the reference group,a total of 100 patients;nitroprusside 100 patients treated with sodium combined with nitroglycerin were included in the experimental group.The treatment effect was evaluated based on the improvement of clinical symptoms and the results of clinical verification,the changes of serum biochemical test indicators were observed,and the incidence of toxic and side effects was recorded.Results The total effective rate of treatment in the experimental group was significantly higher than that in the reference group(88.00%vs.66.00%),the difference was statistically significant(P<0.05).The serum H-FABP,sST2,hs-CRP and other indicators improved significantly,the difference was statistically significant(P<0.05).The improvement of heart failure symptoms took a short time[(25.80±2.10)min vs.(43.60±2.50)min],the difference was statistically significant(P<0.05).The incidence of side effects is relatively low(7.00%vs.15.00%),the difference was statistically significant(P<0.05).Conclusion Compared with the simple use of sodium nitroprusside,the combination of sodium nitroprusside and nitroglycerin has many advantages in acute heart failure,such as rapid effect,protection of myocardial cells,promotion of cardiac function recovery,and high safety.
