摘要
目的探讨药学门诊签约方式用于2型糖尿病管理的成效。方法选取某院2020年4月至2021年3月收治的2型糖尿病患者100例,随机分为干预组和对照组,各50例。两组患者均予常规降血糖治疗。干预组患者进行药学门诊签约,药师每月以门诊随访或电话随访的方式了解其服药情况及血糖、血脂等相关指标的控制情况,并与医师共同制订个体化治疗方案;通过诊间教育,药物治疗评价,制订用药建议、用药评估、药物重整、调整生活方式等药物治疗相关计划,为其制作门诊药历;制订当前的管理目标,每月门诊随访时评价管理效果,根据达标情况制订新的目标或考虑是否调整原目标。比较治疗前后两组患者的糖、脂代谢指标及自我管理水平。结果干预组患者治疗后的糖化血红蛋白、空腹血糖、餐后2 h血糖、三酰甘油、总胆固醇水平均明显低于对照组,用药依从性、饮食控制、运动规律比例均明显高于对照组(P<0.05)。结论药学门诊签约方式用于2型糖尿病管理成效良好,具有一定可行性。
Objective To investigate the effectiveness of contract management of type 2 diabetes mellitus(T2 DM)in the pharmaceutical outpatient clinic.Methods A total of 100 patients with T2 DM admitted to the hospital from April 2020 to March 2021 were selected and randomly divided into the intervention group and the control group,50 cases in each group.The patients in the two groups were given routine hypoglycemic treatment,on this basis,the patients in the intervention group were given contract management of T2 DM in the pharmaceutical outpatient clinic.Pharmacists mastered the patients’medication and the control of blood glucose level,blood lipid and other related indicators through outpatient follow-up or telephone follow-up every month to formulate an individualized treatment plan with doctors,they formulated drug treatment-related plans such as drug suggestion,drug evaluation,drug reorganization and lifestyle adjustment through inter clinic education and drug treatment evaluation to formulate an individualized treatment plan and outpatient drug calendar for patients,formulated the current management objectives,evaluated the management effect during monthly follow-up,and formulated new objectives according to the compliance or consider whether to adjust the original objectives.The glucose and lipid metabolism indexes and self-management ability of the two groups were compared before and after treatment.Results The levels of glycosylated hemoglobin,fasting blood glucose(FBG),2 h postprandial blood glucose(2 hPG),triglyceride(TG)and total cholesterol(TC)in the intervention group were significantly lower than those in the control group,and the medication compliance,dietary control ability and exercise regularity in the intervention group were significantly higher than those in the control group(P<0.05).Conclusion The application of contract management mode in the pharmaceutical outpatient clinic is effective in the management of T2 DM,which is feasible.
