摘要
目的:探讨抑郁障碍(MDD)患者在Thinc-it工具下的认知功能损害特点及与临床症状间的关系。方法:应用汉密尔顿抑郁量表(HAMD-17)、汉密尔顿焦虑量表(HAMA)、30项抑郁症状自评问卷(IDS-SR30)对47例MDD急性发作患者(MDD组)的临床症状进行评估;采用Thinc-it工具测试MDD患者的认知功能,其中客观测试部分为选择反应时间(CRT)、1-Back记忆任务(1-Back)、数字符号替代测试(DSST)及连线测试B(TMT-B);主观测试部分为认知损害5项问卷(PDQ-5-D);结果与34名健康志愿者(HC组)比较;采用Pearson相关性分析MDD患者认知功能损害及与临床症状间的关系。结果:①与HC组相比,MDD组在CRT和TMT-B标准分明显下降,CRT平均反应时间及TMT-B总完成时间明显延长(P<0.05);PDQ-5-D标准分显著降低(P<0.01);②MDD患者CRT标准分与HAMA中躯体性焦虑因子分呈负相关,TMT-B标准分与IDS-SR30中睡眠障碍因子分呈负相关,PDQ-5-D标准分数与IDS-SR30总分、IDS-SR30中情绪/认知及焦虑/唤醒因子分呈负相关(P<0.05或P<0.01)。结论:在Thinc-it工具测试下,MDD患者存在注意力、执行功能以及主观认知功能方面的缺陷,且与情绪、焦虑及睡眠改变密切相关。
Objective:To investigate characteristics of cognitive impairment and its relationship with clinical symptoms in patients with major depressive disorder(MDD)under Thinc-integrated Tool(Thinc-it).Method:The 17-item Hamilton Depression Scale(HAMD-17),Hamilton anxiety scale(HAMA)and Self-rated Inventory of Depressive Symptomatology(IDS-SR30)were used to evaluate the clinical symptoms of 47 patients with acute episode of MDD.And Thinc-it was used to test the cognitive function of patients with MDD.The objective test included selection response time(CRT),1-back memory task(1-back),digit symbol substitution test(DSST)and line test B(TMT-B).And the subjective part of the test was a 5-item questionnaire for cognitive impairment(PDQ-5-D).Cognitive function test results were compared with 34 healthy volunteers(HC group).Pearson correlations were used to investigate the relationship between cognitive impairment and clinical symptoms in MDD patients.Results:Compared with HC group,the standard scores of CRT and TMT-B in MDD group significantly reduced,while the mean reaction time of CRT and the total completion time of TMT-B significantly prolonged(P<0.05).PDQ-5-D standard score significantly decreased(P<0.01).In terms of relevance,the CRT standard score was negatively correlated with HAMA somatic anxiety factor score.The TMT-B standard score was negatively correlated with IDS-SR30 sleep factor score.The PDQ-5-D standard score was negatively correlated with the total score of IDS-SR30,and the scores of IDS-SR30 mood/cognition and anxiety/arousal factor(P<0.05 or P<0.01).Conclusion:Attention,executive function and objective cognitive function were impaired in patients with MDD and associated with mood,anxiety and sleep changes using Thinc-it tool.
