摘要
目的探讨小剂量日间非卧床腹膜透析治疗糖尿病肾病尿毒症(DNU)的效果。方法抽取2015年6月至2020年6月大同市第三人民医院收治的DNU患者60例, 按照治疗方案分为两组。对照组30例行小剂量持续非卧床腹膜透析方案, 观察组30例行小剂量日间非卧床腹膜透析方案。治疗半年后评价效果, 比较两组残肾功能、血糖水平、营养状态。结果治疗后, 观察组残肾腹膜尿素清除率(Kt/V)、血清白蛋白、血红蛋白高于对照组(t=2.412、3.557、3.036, P<0.05), 观察组24 h尿蛋白量、空腹血糖、糖化血红蛋白、24 h腹膜液蛋白低于对照组(t=2.337、2.964、3.008、2.903, P<0.05);两组腹膜Kt/V比较, t=0.097, P>0.05。结论 DNU患者选择小剂量日间非卧床腹膜透析治疗, 可更好地保护残肾功能, 改善血糖水平, 维持良好的营养状态。
Objective To investigate the efficacy of small-dose day ambulatory peritoneal dialysis on diabetic nephropathy with uremia(DNU).Methods A total of 60 DNU patients admitted to Datong Third People’s Hospital from June 2015 to June 2020 were selected and divided into two groups according to the dialysis plan.Thirty patients in the control group underwent low-dose continuous ambulatory peritoneal dialysis,and 30 patients in the observation group underwent low-dose day ambulatory daytime peritoneal dialysis.The effect was evaluated after half a year of treatment,and the residual kidney function,blood sugar level,and nutritional status of the two groups were compared.Results After treatment,the residual kidney peritoneal clearance of urea(Kt/V),serum albumin and hemoglobin in the observation group were higher than those in the control group(t=2.412,3.557,3.036,P<0.05).The 24-hour urine protein content,fasting blood glucose,glycosylated hemoglobin,and 24-hour peritoneal fluid protein in the observation group were lower than those in the control group(t=2.337,2.964,3.008,2.903,P<0.05);the peritoneal Kt/V were compared between the two groups,t=0.097,P>0.05.Conclusions Patients with DNU who choose low-dose day ambulatory peritoneal dialysis treatment can better protect residual kidney function,improve blood sugar levels,and maintain good nutritional status.
