血液分析仪和镜检法测定白细胞计数及分类的结果比较

摘要 目的探究血液分析仪法和镜检法测定白细胞计数及分类的结果。方法收集2020年1—10月于本院行血常规检查的115例患者的血液标本,均采用血液分析仪法和镜检法(金标准)检测白细胞计数,比较两种方法的检测结果。结果115例患者血液样本中,镜检法检测的白细胞计数水平为(7.12±2.37)×10^(9)/L,血液分析仪法为(7.10±2.34)×10^(9)/L,两者比较差异无统计学意义。镜检法检测白细胞计数阳性24例,阴性91例;血液分析仪法检测白细胞计数阳性29例,阳性率为25.22%(29/115)。血液分析仪法检测白细胞计数的准确性为92.17%(106/115)、灵敏度为91.67%(22/24)、特异度为92.31%(84/91)、阳性预测值为75.86%(22/29)、阴性预测值为97.67%(84/86)。血液分析仪法异常细胞检出率高于镜检法,差异有统计学意义(P<0.05)。结论血液分析仪检测白细胞计数的效果较好,但单独应用无法达到镜检法对各类细胞的识别、分类控制效果,存在一定误诊风险。 Objective To explore the results of leukocyte count and classification measured by blood analyzer and microscopic examination.Methods The blood samples of 115 patients who underwent routine blood examination in our hospital from January to October 2020 were collected,and then the blood analyzer method and microscopic examination method(gold standard)were used for leukocyte count detection,and the detection results of the two methods were compared.Results In the blood samples of 15 patients,the white blood cell count level detected by microscopic examination was(7.12±2.37)×10^(9)/L,and that by blood analyzer was(7.10±2.34)×10^(9)/L,and there was no significant difference between the two methods.Microscopic examination showed that 24 cases were positive and 91 cases were negative.29 cases were found to be positive by blood analyzer,and the positive rate was 25.22%(29/115).The accuracy,sensitivity,specificity,positive predictive value and negative predictive value of leukocyte count by blood analyzer were 92.17%(106/115),91.67%(22/24),92.31%(84/91),75.86%(22/29)and 97.67%(84/29),respectively.The detection rate of abnormal cells by hematology analyzer was higher than that by microscopy,and the difference was statistically significant(P<0.05).Conclusion The blood analyzer has good effect in white blood cell count detection,but the microscopic examination can't realize the identification and classification control of all kinds of cells by instrument analysis alone,and there is certain risk of misdiagnosis.
