摘要
目的探讨盐酸氨溴索联合布地奈德治疗新生儿肺炎的疗效。方法回顾性分析2017年5月至2020年1月本院收治的80例肺炎患儿的临床资料,根据治疗方法的不同分为对照组与观察组,每组40例。对照组采用盐酸氨溴索治疗,观察组在对照组基础上联合布地奈德治疗。比较两组症状改善时间、炎症因子水平[血清C反应蛋白(CRP)、降钙素原(PCT)]、动脉血气指标[动脉血二氧化碳分压(PaCO_(2))、动脉血氧分压(PaO_(2))]及心肌酶谱变化情况[乳酸脱氢酶(LDH)、肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)]。结果观察组热退时间、肺部湿啰音消失时间、咳嗽消失时间及喘息消失时间均短于对照组,差异有统计学意义(P<0.05)。治疗后,两组CRP、PCT水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。治疗后,两组PaCO_(2)水平均低于治疗前,PaO_(2)水平均高于治疗前,且观察组PaCO_(2)水平低于对照组,PaO_(2)水平高于对照组,差异有统计学意义(P<0.05)。治疗后,两组LDH、CK、CK-MB水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。结论盐酸氨溴索联合布地奈德治疗新生儿肺炎效果显著,可促进患儿恢复,控制炎症反应,调节动脉血气,减轻心肌损伤。
Objective To investigate the efficacy of ambroxol hydrochloride combined with budesonide in the treatment of neonatal pneumonia.Methods The clinical data of 80 children with pneumonia admitted to our hospital from May 2017 to January 2020 were retrospectively analyzed.According to different treatment methods,they were divided into control group and observation group,with 40 cases in each group.The control group was treated with ambroxol hydrochloride,and the observation group was treated with budesonide on the basis of the control group.The symptom improvement time,inflammatory factor levels[serum C-reactive protein(CRP),procalcitonin(PCT)],arterial blood gas indexes[arterial partial pressure of carbon dioxide(PaCO_(2)),arterial partial pressure of oxygen(PaO_(2))]and myocardial enzyme spectrum changes[lactate dehydrogenase(LDH),creatine kinase(CK),creatine kinase isoenzyme(CK-MB)]were compared between the two groups.Results The fever retreat time,pulmonary rales disappearance time,cough disappearance time and wheezing disappearance time in the observation group were shorter than those in the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of CRP and PCT in the two groups were lower than those before treatment,and the observation group were lower than the control group,the difference was statistically significant(P<0.05).After treatment,the levels of PaCO_(2) in the two groups were lower than those before treatment,and the levels of PaO_(2) were higher than those before treatment,the PaCO_(2) level in the observation group was lower than that of the control group,and the PaO_(2) level was higher than that of the control group,the difference was statistically significant(P<0.05).After treatment,the levels of LDH,CK and CK-MB in the two groups were lower than those before treatment,and the observation group were lower than the control group,the difference was statistically significant(P<0.05).Conclusion Ambroxol hydrochloride combined with budesonide has significant
