摘要
目的:探讨急性时相反应蛋白及肝纤维化四项检测在乙肝肝硬化中的表达及与疾病严重程度的关系。方法:选取2019年10月-2020年12月本院120例乙肝肝硬化患者为观察组,另选取同时期的120例乙肝无肝硬化患者为对照组。检测并比较两组的急性时相反应蛋白(Fib、PA及ALB)及肝纤维化指标(PCⅢ、Ⅳ-C、LN及HA);比较观察组中不同Child-Pugh分级患者的上述指标;采用Spearman秩相关分析上述指标与乙肝肝硬化Child-Pugh分级的关系。结果:观察组PCⅢ、Ⅳ-C、LN及HA均高于对照组(P<0.05)。观察组Fib、PA及ALB均低于对照组(P<0.05)。观察组中Child-Pugh分级C级患者的PCⅢ、Ⅳ-C、LN及HA均高于A、B级者,且B级者均高于A级者(P<0.05);观察组中Child-Pugh分级C级患者的Fib、PA及ALB均低于A、B级者,且B级者均低于A级者(P<0.05)。Spearman秩相关分析显示,各项肝纤维化指标均与乙肝肝硬化Child-Pugh分级呈正相关,各项急性时相反应蛋白均与乙肝肝硬化Child-Pugh分级呈负相关(P<0.05)。结论:乙肝肝硬化患者的急性时相反应蛋白及肝纤维化四项呈现异常表达状态,且与乙肝肝硬化严重程度有密切的关系,检测价值较高。
Objective:To investigate the expression of acute phase reactive protein and four tests of liver fibrosis in hepatitis B cirrhosis and their relationship with disease severity.Method:A total of 120 patients with hepatitis B cirrhosis from October 2019 to December 2020 in our hospital were selected as the observation group,and another 120 hepatitis B patients without cirrhosis during the same period were selected as the control group.The acute phase reactive protein(Fib,PA and ALB)and liver fibrosis index(PCⅢ,Ⅳ-C,LN and HA)were detected and compared between the two groups.The above indexes of patients with different Child-Pugh classifications in the observation group were compared.Spearman rank correlation was used to analyze the relationship between the above indexes and child-Pugh classifications of hepatitis B cirrhosis.Result:PCⅢ,Ⅳ-C,LN and HA in observation group were higher than those in control group(P<0.05).Fib,PA and ALB in observation group were lower than those in control group(P<0.05).In the observation group,PCⅢ,Ⅳ-C,LN and HA of Child-Pugh classifications C patients were higher than those of grade A and B,and those of grade B were higher than those of grade A(P<0.05).In the observation group,the Fib,PA and ALB of the Child-Pugh classifications C patients were lower than those of grade A and B,and those of grade B were lower than those of grade A(P<0.05).Spearman rank correlation analysis showed that all fibrosis indexes were positively correlated with Child-Pugh classifications of hepatitis B cirrhosis,and all acute phase reactive proteins were negatively correlated with Child-Pugh classifications of hepatitis B cirrhosis(P<0.05).Conclusion:The acute phase reactive protein and four tests of liver fibrosis of patients with hepatitis B cirrhosis show abnormal expression state,and they have close relationship with the severity of hepatitis B cirrhosis,so the detection value in the patients with hepatitis B cirrhosis is higher.
