摘要
目的探讨冠状动脉粥样硬化性心脏病(CHD)患者血清白细胞衍生趋化因子2(LECT2)水平及其影响因素。方法纳入2019年5月至2020年5月就诊于四川省人民医院的100例CHD患者为CHD组,同时选取年龄和性别匹配的66例健康体检者为对照组。测量两组身高、体重、血压,测定空腹血糖(FPG)、血脂、胰岛素、LECT2、超敏C反应蛋白(hs-CRP),计算稳态模型胰岛素抵抗指数(HOMA2-IR)。结果 CHD组血清LECT2水平(29.45±6.79)mg/L明显高于对照组(25.17±5.96)mg/L(P<0.05)。Spearman相关分析显示,CHD患者血清LECT2水平与体重指数(BMI)、收缩压(SBP)、FPG、空腹胰岛素(FINS)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-c)、hs-CRP和HOMA2-IR呈正相关(P<0.05);与高密度脂蛋白胆固醇(HDL-c)呈负相关(P<0.05);LECT2与年龄、性别、舒张压(DBP)和总胆固醇(TC)比较,差异无统计学意义(P>0.05)。多元线性回归分析显示,BMI、HDL-c、hs-CRP和HOMA2-IR是CHD患者血清LECT2的独立影响因素。结论 CHD患者血清LECT2水平升高,与CHD的危险因素如糖脂代谢紊乱、慢性炎症密切相关,提示LECT2在CHD的发生发展中可能起到重要作用。
Objective To investigate serum level of leukocyte cell-derived chemotaxin 2(LECT2) in patients with coronary atherosclerotic heart disease(CHD) and its influencing factors. Methods A total of 100 CHD patients in Sichuan Provincial People’s Hospital from May 2019 to May 2020 were included in the experimental group, and 66 age and gender matched physical examinees were enrolled in the control group. Height, weight and blood pressure were measured. Fasting plasma glucose(FPG), blood lipid, insulin, LECT2, high-sensitivity C-reactive protein(hs-CRP) were detected. And homeostasis model assessment of insulin resistance(HOMA2-IR) was calculated. Results Serum LECT2 level in experimental group[(29.45±6.79) mg/L]was significantly higher than that in control group[(25.17±5.96) mg/L](P<0.05). Spearman correlation analysis showed that serum LECT2 level in CHD patients was positively correlated with body mass index(BMI), systolic blood pressure(SBP), FPG, fasting insulin(FIN), triglyceride(TG), low density lipoprotein cholesterol(LDL-c), hs-CRP and HOMA2-IR(P<0.05), negatively correlated with high density lipoprotein cholesterol(HDL-c)(P<0.05), and not significantly correlated with age, sex, diastolic blood pressure(DBP) and total cholesterol(TC)(P>0.05). Multiple linear regression analysis showed that BMI, HDL-c, hs-CRP and HOMA2-IR were independent influencing factors of serum LECT2 in CHD patients. Conclusion The elevated serum LECT2 level in CHD patients is closely related to the risk factors of CHD, such as glucose and lipid metabolism disorder and chronic inflammation, suggesting that LECT2 may play an important role in the occurrence and development of CHD.
