摘要
目的探讨新型冠状病毒肺炎(COVID-19)患者初诊时血液学检测指标变化,为临床诊疗提供参考依据。方法将该院在2020年1月23日至3月1日期间收治的新型冠状病毒肺炎患者纳为COVID-19组(n=15例),同期收治的急性呼吸道感染(ARI)患者纳为ARI组(n=18例),收集并比较两组患者的血常规、凝血功能、炎症指标和生化指标的变化。结果COVID-19组患者的白细胞(WBC)、淋巴细胞(LYM)、碱性磷酸酶(ALP)、清蛋白(ALB)均低于ARI组,差异均有统计学意义(P<0.05);而C-反应蛋白(CRP)、红细胞沉降率(ESR)、白细胞介素-6(IL-6)、天门冬氨酸氨基转移酶(AST)、乳酸脱氢酶(LDH)均高于ARI组,差异均有统计学意义(P<0.05);两组患者的NEU、红细胞(RBC)、HB、血小板(PLT)、凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)、纤维蛋白原(FIB)、D-二聚体(D-D)、丙氨酸氨基转移酶(ALT)、总蛋白(TP)、血尿素氮(BUN)、尿酸(UA)、肌酐(CRE)、肌红蛋白(MYO)、肌酸激酶(CK)等指标比较差异均无统计学意义(P>0.05)。结论新型冠状病毒肺炎患者血液学早期变化表现为WBC正常或降低,LYM降低,CRP、ESR、IL-6升高,患者可出现血液高凝、肝功能受损及心肌酶异常。
Objective To investigate the changes of hematology test indexes of patients with coronavirus disease 2019(COVID-19)during initial diagnosis to provide scientific reference for clinical diagnosis and treatment.Methods The patients with coronavirus disease 2019 admitted in the First People′s Hospital of Foshan were enrolled in group COVID-19(n=15 cases)from Jan 23 to Mar 1,2020.The patients with acute respiratory infection treated in the same period were enrolled in group ARI(n=18 cases).And then compared the differences of blood routine test,coagulation function,inflammation index and biochemical index between the two groups.Results Compared with group ARI,WBC,LYM,ALP,ALB in group COVID-19 were lower than those in group ARI,the differences were statistically significant(P<0.05);while the CRP,ESR,IL-6,AST,LDH in group COVID-19 were higher than those in group ARI,the differences were statistically significant(P<0.05).And there was no significant difference in NEU,RBC,HB,PLT,PT,APTT,TT,FIB,D-D,ALT,TP,BUN,UA,CRE,MYO,CK between the two groups,the differences were not statistically significant(P>0.05).Conclusion In this study,the early changes of hematology test indexes of patients with COVID-19 showed that WBC was normal or decreased,LYM was decreased,the CRP,ESR and IL-6 were increased,and the patients could have hypercoagulability,impaired liver function and abnormal myocardial enzymes.
