摘要
目的:研究中医镇痛灸在冠心病心绞痛患者疼痛护理管理中的应用效果。方法:选取本院2017年8月-2020年8月诊治的154例冠心病心绞痛患者,按照投掷法随机分为75例常规组与79例中医组。常规组给予常规护理,中医组则在此基础上配合镇痛灸疼痛护理。观察并记录两组住院天数、卧床休息天数、心绞痛持续用时及生活质量状况,比较两组干预前、后的心肌相关指标[心肌肌钙蛋白(cTnI)、肌酸激酶同工酶(CK-MB)]及症状缓解状况。结果:中医组卧床休息天数及心绞痛持续用时均比常规组低(P<0.05);两组干预后cTnI、CK-MB较干预前均明显下降,且中医组各指标均比常规组低(P<0.05);两组干预后心悸、胸痛、不寐、神疲乏力评分均较干预前降低,且中医组各评分比常规组低(P<0.05);中医组治疗满意度、躯体受限度、心绞痛发作状况、心绞痛稳定状况评分均比常规组高(P<0.05)。结论:中医镇痛灸在冠心病心绞痛患者疼痛护理管理中疗效较好,能明显缓解患者心绞痛,缩短卧床休息天数,改善生活质量。
Objective: To study the application effect of traditional Chinese medicine(TCM) analgesic moxibustion in pain management for patients with coronary heart disease and angina pectoris. Methods: A total of 154 patients with coronary heart disease and angina pectoris diagnosed and treated in the hospital from August 2017 to August 2020 were enrolled in the study. They were randomly divided into a routine group(75 cases) and a TCM group(79 cases). The routine group were given routine nursing, while the TCM group were given pain nursing via analgesic moxibustion on the basis of routine nursing. The length of hospital stay, time in bed, the duration of angina pectoris, and quality of life of the two groups were recorded. The myocardium associated indicators [cardiac troponin(cTnI),creatine kinase isoenzyme(CK-MB)] before and after intervention, and symptom relief were compared between the two groups. Results: The length of hospital stay and duration of angina pectorisin the TCM group were shorter than those in the routine group(P < 0.05). After intervention, cTnI and CK-MB decreased significantly in the two groups, which were lower in the TCM group than in the routine group(P < 0.05). After the intervention, the scores for palpitations, chest pain, insomnia, and fatigue decreased in the two groups, and the TCM group had lower scores thanthe routine group(P < 0.05). The TCM group had higher scores than the routine group in terms of satisfaction with treatment, physical limitation, anginaonset and stable angina pectoris(P < 0.05). Conclusion: TCM analgesic moxibustion has a good effect in pain management of patients with coronary heart disease and angina pectoris. It can significantly relieve angina pectoris, shorten the time of bed rest, and improve quality of life.
