摘要
目的:探讨孕激素作用下子宫内膜厚度的变化是否对冷冻胚胎移植周期的妊娠结局有影响。方法:回顾性分析2013年1月至2020年7月期间在我中心行首次冻融胚胎移植周期(FET)治疗1378例,在内膜转化日和胚胎移植当日通过阴道彩超对子宫内膜厚度进行测量,并记录子宫内膜厚度变化,追踪患者的临床妊娠率(CPR)和活产率(LBR)。结果:无论采用自然周期(NC)还是人工周期(EP)等子宫内膜制备方案,与内膜转化日相比,子宫内膜厚度在FET日可能会增加、减少或保持不变。在自然周期中,子宫内膜厚度的增加组、不变组、减少组的临床妊娠率分别为42.86%、46.28%、43.67%,而人工周期中,子宫内膜厚度的增加组、不变组、减少组的临床妊娠率分别为52.44%、48.15%、53.49%,显示三组之间在两个周期中均无显著差异(P=0.82;P=1.0)在自然周期中,子宫内膜厚度的增加组、不变组、减少组的活产率分别为35.14%、35.5%、33.73%,而人工周期中,子宫内膜厚度的增加组、不变组、减少组的活产率分别为41.6%、33.33%、41.08%,显示三组之间在两个周期中均无显著差异(P=0.91;P=0.382)。结论:与内膜转化日相比,子宫内膜厚度在FET当日可能增加、减少或保持不变。服用孕酮后子宫内膜厚度的任何变化都不会对FET周期中的临床妊娠率和活产率产生显著影响。
Objective:To evaluate whether endometrial thickness(EMT)change in response to progesterone has an effect on pregnancy outcomes in frozen-thawed embryo transfer(FET)cycles.Methods:This observational study included 1378 patients undergoing their first FET cycles in our center between January 2013 and July 2020.EMT was measured by transvaginal ultrasound on the day of progesterone administration and on the day of FET.The change in EMT was recorded.The clinical pregnancy rate(CPR)and the live birth rate(LBR)were followed up in these patients.Results:Regardless of endometrial preparation protocol by natural cycle(NC)or estrogen-progesterone replacement therapy(EP),EMT may increase,decrease or remain stable on the day of FET compared with that on the day of progesterone administration.The CPRs in patients with increased,stable or decreased EMT were 42.86%,46.28% and 43.67% by NC cycle vs 52.44%,48.15%,and 53.49% by EP,with no significant difference among the three groups by either cycle(P=0.82 and P=1.0).The LBRs in patients with increased,stable or decreased EMT were 35.14%,35.5%,and 33.73% by NC vs 41.6%,33.33%,and 41.08% by EP,with no significant difference among the three groups by either cycle(P=0.91 and P=0.382).Conclusion:EMT may increase,decrease or remain stable on the day of FET as compared with that on the day of progesterone administration.Whatever change in EMT that occurs after progesterone administration has no significant effect on CPR and LBR in FET cycles.
