摘要
目的探讨电子计算机断层扫描(CT)血管造影联合心脏超声对法洛四联症(TOF)先天血管畸形术前诊断中的应用价值。方法回顾性选取2018年1月至2020年7月江西省儿童医院80例确诊TOF的患儿作为研究对象,进行心脏超声与64排螺旋CT血管造影检查,应用多种图像后处理技术进行图像重建,结合心脏超声和临床手术结果,评估CT血管造影联合心脏超声在TOF先天血管畸形术前诊断中的应用价值。结果CT血管造影联合心脏超声诊断TOF先天血管畸形的敏感度高于心脏超声和CT血管造影单独诊断,差异有统计学意义(P<0.05);CT血管造影联合心脏超声诊断TOF先天血管畸形的准确度高于心脏超声单独诊断,差异有统计学意义(P<0.05);三种检查方式一致性良好(Kappa=0.468,0.451,0.628)。结论CT血管造影联合心脏超声为临床提供准确可靠的影像依据,减少不必要的创伤性检查,为手术方案的制定提供有利支撑。
Objective To explore the application value of computerized tomography(CT)angiography combined with cardiac ultrasound in the preoperative diagnosis of congenital vascular malformation of tetralogy of fallot(TOF).Methods A total of 80 children patients with confirmed TOF in Jiangxi Provincial Children′s Hospital between January 2018 and July 2020 were selected as the research object retrospectively,fcardiac ultrasound and 64 rows of spiral CT angiography,the application of multiple image postprocessing technology for image reconstruction,combined with cardiac ultrasound and clinical surgery results,and the value of CT angiography combined with cardiac ultrasound in the preoperative diagnosis of TOF congenital vascular malformations was evaluated.Results The sensitivity in the diagnosis of CT angiography combined with cardiac ultrasound were higher than that in the separate diagnosis of cardiac ultrasound and CT angiography,with statistical significant differeces(P<0.05).And the accuracy in the diagnosis of CT angiography combined with cardiac ultrasound were higher than that in the separate diagnosis of cardiac ultrasound,with statistical significant differeces(P<0.05).The three inspection methods were very consistent(Kappa=0.468,0.451,0.628).Conclusion CT angiography combined with cardiac ultrasound can provide accurate and reliable imaging basis for the clinic,reduce unnecessary traumatic examinations,and provide favorable support for the formulation of surgical plans.
