摘要
目的探讨血清促生长激素释放肽(Ghrelin)、血栓素蛋白1(TSP-1)及和肽素(Copeptin)在慢性心力衰竭(CHF)患者中的检测意义及与心血管终点事件的关系。方法选取180例CHF患者,根据患者NYHA心功能分级分为Ⅱ级(n=75)、Ⅲ级(n=62)、Ⅳ级(n=43),另选取同期健康体检者60人作为对照组,对比各组临床基本资料、心功能、实验室指标;受试者工作特征曲线(ROC)分析Ghrelin、TSP-1及Copeptin水平对于CHF心血管终点事件的诊断价值。结果各组心率(HR)、右心室前后径(RVD)、右心室心肌做功指数(RV-MPI)、左心房前后径(LAD)、左心室舒张末内径(LVEDD)、左心室射血分数(LVEF)、三尖瓣环收缩期位移(TAPSE)、血糖、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、尿酸(UA)、氨基末端脑钠肽前体(NTproBNP)、C反应蛋白(CRP)、干扰素-γ(IFN-γ)、白介素-4(IL-4)、Ghrelin、TSP-1及Copeptin比较差异有统计学意义(P<0.05)。相关性分析结果显示,Ghrelin、TSP-1及Copeptin分别与NYHA分级、高血压病史、糖尿病史、冠心病史、TC、TG、UA、NT-proBNP、IL-4、CRP、IFN-γ存在正相关性(P<0.05)。发生心血管终点事件组Ghrelin、TSP-1及Copeptin显著高于未发生组(P<0.05)。ROC曲线结果显示,Ghrelin、TSP-1和Copeptin单独检测对于CHF并发心血管终点事件诊断的AUC分别为0.817、0.912、0.890,三者联合检测的AUC为0.933,三者联合诊断具有显著价值。结论检测血清Ghrelin、TSP-1及Copeptin水平有助于了解慢性心力衰竭病情发展。
Objective To investigate the significance of serum growth hormone releasing peptide(Ghrelin),thromboxane protein 1(TSP-1)and copeptin(Copeptin)in patients with chronic heart failure(CHF)and cardiovascular endpoint events relationship. Methods 180 CHF patients were selected and classified into grade Ⅱ(n=75),grade Ⅲ(n=62),grade Ⅳ(n=43)according to the NYHA cardiac function classification. In addition,60 healthy patients in the same period were selected as the control group. The basic clinical data,cardiac function,and laboratory indicators of each group were compared. The receiver operating characteristic curve(ROC)was analyzed for the diagnostic value of Ghrelin,TSP-1 and Copeptin levels for CHF cardiovascular endpoint events. Results Heart rate(HR),anteroposterior diameter of right ventricle(RVD),right ventricle myocardial work index(RV-MPI),left atrium diameter(LAD),left ventricle end-diastolic diameter(LVEDD),left ventricular ejection fraction(LVEF),tricuspid annulus plane systolic excursion(TAPSE),blood glucose,total cholesterol(TC),triglyceride(TG),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),uric acid(UA),N-terminal pro-brain natriuretic peptide(NT-proBNP),C-reactive protein(CRP),interferon-γ(IFN-γ),interleukin-4(IL-4),Ghrelin,TSP-1 and copeptin were significantly different(P<0.05).Correlation analysis results showed that Ghrelin,TSP-1 and Copeptin were positively correlated with NYHA classification,history of hypertension,diabetes,coronary heart disease,TC,TG,UA,NT-proBNP,IL-4,CRP and IFN-γ(P<0.05).Ghrelin,TSP-1 and Copeptin were significantly higher in the group with cardiovascular endpoint events(P<0.05). The ROC curve results showed that the AUC of Ghrelin,TSP-1 and Copeptin for the diagnosis of CHF complicated cardiovascular endpoint events were 0.817,0.912 and 0.890,respectively,and the combined detection AUC was 0.933.The combined diagnosis of the three had significant value.
