摘要
目的探讨螺内酯联合美托洛尔对心房纤颤患者运动耐力及左心室射血分数的影响。方法选取2018年2月-2019年10月扶沟县人民医院心血管内科收治的心房纤颤患者90例,根据随机数字表法分为观察组(45例)和对照组(45例)。两组均给予美托洛尔治疗,基于此,观察组联合螺内酯治疗,比较两组运动耐力、心功能及不良反应发生情况。结果治疗后,两组6min步行距离均较治疗前增加,且观察组较对照组高,差异有统计学意义(P<0.05);治疗后,两组左心室射血分数(LVEF)较治疗前增加,左心室收缩末期容积(LVESV).左心室舒张末期容积(LVEDV)较治疗前降低,且观察组LVEP高于对照组,LVBSV、LVEDV低于对照组,差异有统计学意义(P<0.05);治疗后,两组每搏输出量(SV)、心脏指数(CI)、心排血量(CO)水平均较治疗前升高,且观察组较对照组高,差异有统计学意义(P<0.05);两组不良反应发生率对比,差异无统计学意义(P>0.05).结论螺内酯联合美托洛尔治疗心房纤颤疗效显著,可有效提高患者运动耐力,改善心功能,且联合治疗不增加患者的不良反应,安全性较高。
0bjective To investigate the effect of spironolactone combined with mettolol on motor endurance and left ventricular ection fraction in patients with atrial firllaion.Mlethods 90 patients with atrial firilation adnitted to Cardiovascular Department of Fugou County People's Hospital from February 2018 to October 2019,and were divided into observation groups(45 cases)and control groups(45 cases)according to the randomized digital table.Both groups were treated with metolol,based on which the abservation group was treated with spironolactone to compare exercise endurance,cardiac function and adverse effects.Results After the trealment,Both groups had an elevated 6min walking distance compared with before treatment,And the observaion group was higher than the control group,The diference was stitical significant(P<0.05);post-treatment,The left ventricular ejection fraction(LVEF)was increased compared with pre-treatmenl,Left ventricular end-systolic volume(LVESV)and left ventricular end-diastolic volume(LVEDV)were reduced compared with pre-treatment,And the LVEF was higher than the control group,LVESV and LVEDV were lower than those in the control group,The diference was stistial significant(P<0.05);post-teatment,The output of each beat(SV),cardiac index(CD)and cardiac output(CO)were increased compared with before trealmenl,And the observation group was higher than the control group,The diference was statistical significant(P<0.05);Comparison of adverse efcts incidence between the two groups,The difference was not signifcantly significant(P>0.05).Conclus ion Spironolactone combined with metrolol in atrial firillation,which can improve motor endurance,improve heart function and does not increase adverse ffts.
