摘要
目的:比较症状性颞叶内侧癫痫和颞叶外侧癫痫患者的脑电图特征。方法:选取赣州市人民医院神经内科2019年1月至2020年12月诊治的35例症状性颞叶内侧癫痫患者(内侧组)及30例症状性颞叶外侧癫痫患者(外侧组),两组入院后经长程视频脑电图监测,比较两组患者症状学表现及脑电图特征。结果:两组患者癫痫先兆发作、自动症、对侧肢体强直发作或/和肌张力障碍、继发全面强直阵挛发作等比较,差异无统计学意义(P>0.05)。内侧组患者中多脑叶放电14.3%(5/35例),双侧放电51.4%(18/35例),同步放电11.4%(4/35例),睡眠期放电14.3%(5/35例)。外侧组患者中未发生多脑叶放电及同步化放电,睡眠期放电3.3%(1/30例),双侧放电26.7%(8/30例)。内侧组发作间期脑电图正常者为8.6%,低于外侧组的36.7%(P<0.05);痫样放电与病变部位一致者42.9%,高于外侧组的16.7%(P<0.05),痫样放电与病变部位不同者两组比较差异无统计学意义(P>0.05)。结论:症状性颞叶内侧癫痫与颞叶外侧癫痫典型的症状学表现相一致,脑电图特征差异明显,根据脑电图特征可在一定程度上加以区分。
Objective: To compare the EEG characteristics of symptomatic medial temporal lobe epilepsy and lateral temporal lobe epilepsy. Methods: Thirty-five patients with symptomatic medial temporal lobe epilepsy(medial group) and 30 patients with symptomatic lateral temporal lobe epilepsy(lateral group) were selected from the Department of Neurology of Ganzhou People’s Hospital from January 2019 to December 2020. The patients were monitored by long-term video-EEG, and the symptomatic manifestations and EEG characteristics of the two groups were compared. Results: There was no significant difference between the two groups in aura onset, automatism, contralateral limb tonic seizure or/and dystonia, and secondary generalized tonic-clonic seizure(P>0.05). In the medial group, multiple brain lobes discharge was 14.3%(5/35 cases), bilateral discharge was 51.4%(18/35 cases), synchronous discharge was 11.4%(4/35 cases) and sleep discharge was 14.3%(5/35 cases).In the lateral group, there was no multi-lobar discharge and synchronous discharge, with 3.3%(1/30 cases) of discharge during sleep and 26.7%(8/30 cases) of bilateral discharge. In the medial group, 8.6% of the patients had normal interictal EEG, which was lower than 36.7% in the lateral group(P<0.05);the epileptiform discharge consistent with the lesion site was 42.9%, which was higher than 16.7% in the lateral group(P<0.05). There was no significant difference for epileptiform discharge inconsistent with the lesion site between the two groups(P>0.05).Conclusion: Symptomatic medial temporal lobe epilepsy and lateral temporal lobe epilepsy have the same typical symptomatology, and the EEG features are obviously different, which can be distinguished to some extent according to the EEG features.
