摘要
目的研究临床药师干预辅助用药对药品使用合理性的影响。方法明确2018年7月至2018年12月本院销售金额排名前十的辅助用药,分别选择2018年7月至2018年12月(临床药师干预前)、2019年1月至2019年6月(临床药师干预后)使用这10种辅助用药的病历各1000份。比较干预前、后销售金额排名前十的辅助用药不合理应用情况、出院患者辅助用药处方的不合理情况。结果干预后,销售金额排名前十的辅助用药的用药不合理比例均显著低于干预前(P<0.05)。干预后,辅助用药处方的不合理比例明显低于干预前(P=0.001)。干预后,辅助用药处方的各种不合理情况(适应证不适宜、药品选择不适宜、溶媒选择不适宜、给药途径不适宜、用法用量不适宜、用药疗程过长、存在用药禁忌、联合用药不适宜、重复用药)均少于干预前(P<0.05)。结论临床药师干预辅助用药可使药品使用合理性提高。
Objective To study the influence of clinical pharmacists'intervention in auxiliary medication on the rationality of drug use.Methods The adjuvant medication in the top ten in sales amount of our hospital from July 2018 to December 2018 were identified,and 1000 medical records of these 10 adjuvant medication were respectively selected from July 2018 to December 2018(before clinical pharmacist intervention)and January 2019 to June 2019(after clinical pharmacists'intervention).The irrational application situation of adjuvant medication in the top ten in sales amount and irrational situation of prescription of adjuvant medication for discharged patients before and after intervention were compared.Results After intervention,the irrational proportions of adjuvant medication ranked in the top ten in sales amount were significantly lower than those before intervention(P<0.05).The irrational proportion of prescriptions of adjuvant medication after intervention was 9.50%,which was significantly lower than 26.10%before intervention(P=0.001).After intervention,the irrational situation of prescriptions of adjuvant medication(inappropriate indications,inappropriate drug selection,inappropriate solvent selection,inappropriate route of administration,inappropriate dosage,long course of medication,contraindications,inappropriate combination and repeated medication)were less than those before intervention(P<0.05).Conclusion The clinical pharmacists'intervention in adjuvant medication can improve the rationality of drug use.
