摘要
目的探讨益气活血化瘀汤联合常规治疗对高血压脑出血微创术后患者的临床疗效。方法148例患者随机分为对照组和观察组,每组74例,对照组给予常规治疗(吸氧、控制血压血糖、营养支持等),观察组在对照组基础上加用益气活血化瘀汤,疗程4周。检测临床疗效、颅内压、血肿体积、中医证候评分、血管内皮功能指标(VEGF、ET-1、NO)、S100B、Cys C、神经功能评分(NIHSS评分、ADL评分、GCS评分)变化。结果观察组总有效率显著高于对照组(P<0.05)。治疗后,2组颅内压、血肿体积、中医证候评分、VEGF、NO、ET-1、S100B、Cys C、NIHSS评分降低(P<0.05),ADL评分、GCS评分升高(P<0.05),以观察组更明显(P<0.05)。2组未发现明显不良反应。结论益气活血化瘀汤联合常规治疗可安全有效地改善高血压脑出血微创术后患者临床症状、血管内皮功能、血清S100B和Cys C水平,降低颅内压,缩小血肿体积。
AIM To explore the clinical effects of Yiqi Huoxue Huayu Decoction combined with conventional treatment on patients with hypertensive intracerebral hemorrhage after minimally invasive surgery.METHODS One hundred and forty-eight patients were randomly assigned into control group(74 cases)for 4-week intervention of conventional treatment(oxygen inhalation,control of blood pressure and blood sugar,nutrition support,etc.),and observation group(74 cases)for 4-week intervention of both Yiqi Huoxue Huayu Decoction and conventional treatment.The changes in clinical efficacy,intracranial pressure,hematoma volume,TCM syndrome score,vascular endothelial function indices(VEGF,ET-1,NO),S100 B,Cys C and neurological function scores(NIHSS score,ADL score,GCS score)were detected.RESULTS The observation group demonstrated higher total effective rate than the control group(P<0.05).After the treatment,the two groups displayed decreased intracranial pressure,hematoma volume,TCM syndrome score,VEGF,NO,ET-1,S100 B,Cys C and NIHSS score(P<0.05),and increased ADL score and GCS score(P<0.05),especially for the observation group(P<0.05).No obvious adverse reactions were observable in the two groups.CONCLUSION For the patients with hypertensive intracerebral hemorrhage after minimally invasive surgery,Yiqi Huoxue Huayu Decoction combined with conventional treatment can safely and effectively improve clinical symptoms,vascular endothelial functions and serum S100 B,Cys C levels,decrease intracranial pressure,and reduce hematoma volume.
