骨科患者创口分泌物的细菌检验与耐药性分析

摘要 目的调查研究该院骨科患者创口分泌物的细菌分布情况,并解析其耐药特征,为临床规范诊疗提供科学有效的信息。方法对该院2018年1月-2020年1月骨科送检的148份分泌物进行细菌培养检测及药敏测定,然后进行数据分析。结果经检验,在148份分泌物中共分离出非重复病原菌63株,分离率为42.57%。其中,分离出44株(69.84%)革兰阳性球菌,主要为金黄色葡萄球菌(61.90%)和凝固酶阴性葡萄球菌(7.94%);分离出19株(30.16%)革兰阴性杆菌,主要为大肠埃希菌(9.52%)和阴沟杆菌(9.52%);另有4株(6.35%)其他革兰阴性杆菌。经药敏测定,检出12株(30.77%)耐甲氧西林的金葡菌和4株(80.00%)耐甲氧西林的凝固酶阴性葡萄球菌。所有葡萄球菌对青霉素G均显示出完全耐药。没有检出耐万古霉素和利奈唑胺的葡萄球菌。此外,检出4株(66.67%)产超广谱β–内酰胺酶的大肠埃希菌,并且呈现出多重耐药性特征。未发现对碳青霉烯类耐药的肠杆菌。结论该院骨科患者伤口感染的病原菌呈现多样化,出现一定程度的耐药性,我们应该给予高度重视,加强病原学检测,规范抗菌药物的使用,从而提高诊疗效果,加速患者康复。 Objective To investigate the distribution of bacteria in wound secretions of orthopedic patients in the hospital,and analyze the characteristics of drug resistance,so as to provide scientific and effective information for clinical standard diagnosis and treatment.Methods The 148 secretions submitted by the orthopedics department from January 2018 to January 2020 in the hospital were tested for bacterial culture and drug susceptibility,and then the data was analyzed.Results After testing,a total of 63 non-repetitive pathogens were isolated from 148 secretions,with an isolation rate of 42.57%.Among them,44 strains(69.84%)of Gram-positive cocci were isolated,mainly Staphylococcus aureus(61.90%)and coagulase-negative Staphylococcus(7.94%);19 strains(30.16%)of Gram-negative bacilli were isolated,mainly was Escherichia coli(9.52%)and Cloacae(9.52%);4 strains(6.35%)of other gram-negative bacilli.After drug susceptibility testing,12 strains(30.77%)methicillin-resistant Staphylococcus aureus and 4 strains(80.00%)methicillin-resistant coagulase-negative Staphylococcus were detected.All staphylococci showed complete resistance to penicillin G.No vancomycin and linezolid resistant staphylococci were detected.In addition,4 strains(66.67%)of Escherichia coli producing extended-spectrumβ-lactamases were detected,and they showed the characteristics of multi-drug resistance.No enterobacteria resistant to carbapenems were found.Conclusion The pathogens of wound infections in orthopedic patients in the hospital are diversified and have a certain degree of drug resistance.We should attach great importance to them,strengthen pathogenic testing,and standardize the use of antibacterial drugs,so as to improve the effectiveness of diagnosis and treatment and accelerate the recovery of patients.
