唑来膦酸治疗原发性骨质疏松性椎体压缩性骨折的临床评估 被引量:3

Clinical Evaluation of Zoledronic Acid in the Treatment of Primary Osteoporotic Vertebral Compression Fractures

摘要 目的研究分析唑来膦酸治疗原发性骨质疏松性椎体压缩性骨折的临床评估。方法抽取2018年6月-2019年5月该院收诊的110例老年女性骨质疏松性椎体压缩性骨折患者,依据给药方式的差异将其以1∶1的比例分为两组,给予常规治疗的为对照组(n=55),实施常规治疗和唑来膦酸联合治疗的为观察组(n=55),对比分析两组治疗后疼痛[视觉模拟评分表(VAS)]评分和功能障碍[功能障碍指数评分量表(ODI)]评分和碱性磷酸酶(BAP)、环磷酰胺(CTX)、骨密度水平(BMD)改善程度。结果观察组治疗后VAS、ODI评分分别为(1.52±0.45)分、(20.88±3.45)分,低于对照组的(2.45±0.56)分、(29.55±4.56)分,差异有统计学意义(t=9.601、11.245,P<0.05);观察组治疗后BAP、CTX评分分别为(16.45±3.25)μg/L、(0.41±0.07)μg/L,均低于对照组的(22.77±4.36)μg/L、(0.59±0.11)μg/L,差异有统计学意义(t=8.619、10.238,P<0.05);观察组治疗后的BMD数据为腰椎1~4(0.831±0.049)、股骨颈(0.801±0.079)、Ward三角(0.793±0.051)g/cm^(3),优于对照组的(0.753±0.051)、(0.732±0.071)、(0.736±0.048)g/cm^(3),差异有统计学意义(t=8.179、4.818、6.035,P<0.05)。结论在原发性骨质疏松性椎体压缩性骨折患者的临床用药中使用常规治疗联合唑来膦酸具有良好的效果,不仅可以抑制破骨细胞和溶骨性病变,降低骨吸收,还能增加椎体骨密度有效缓解疼痛,安全有效。 Objective To study and analyze the clinical evaluation of zoledronic acid in the treatment of primary osteoporotic vertebral compression fractures.Methods Selected 110 elderly female patients with osteoporotic vertebral compression fractures admitted to the hospital from June 2018 to May 2019,and divide them into two groups at a ratio of 1:1 according to the difference in administration method:the control group(n=55)was given conventional treatment,and the observation group(n=55)was given conventional treatment and zoledronic acid.The pain[Visual Analogue Scale(VAS)]scores and dysfunction[Dysfunction Index Scale(ODI)]scores,alkaline phosphatase(BAP),cyclop hosphamide(CTX),and bone mineral density(BMD)level of improvement after treatment between the two groups were compared and analyzed.ResultsVAS and ODI scores of the observation group after treatment were(1.52±0.45)points and(20.88±3.45)points,respectively,lower than those of the control group(2.45±0.56)points and(29.55±4.56)points,the difference was statistically significant(t=9.601,11.245,P<0.05).BAP and CTX scores in the observation group were(16.45±3.25)μg/L and(0.41±0.07)μg/L,respectively,which was lower than those in the control group(22.77±4.36)μg/L and(0.59±0.11)μg/L,the difference was statistically significant(t=8.619,10.238,P<0.05).BMD of the observation group after treatment were lumbar 1~4(0.831±0.049)g/cm^(3),femoral neck(0.801±0.079)g/cm^(3),ward triangle(0.793±0.051)g/cm^(3),which was better than the control group(0.753±0.051)g/cm^(3),(0.732±0.071)g/cm^(3),(0.736±0.048)g/cm^(3),the difference was statistically significant(t=8.179,4.818,6.035,P<0.05).Conclusion In clinical treatment of patients with primary osteoporotic vertebral compression fracture,conventional treatment combined with zoledronic acid has a good effect,which can not only inhibit osteoclasts and lytic lesions,reduce bone resorption,but also increase vertebral bone density and effectively relieve pain,which is safe and effective.
