摘要
目的分析产前超声检查与MRI检查对胎盘植入的诊断价值。方法方便选取2017年10月—2019年10月该院收治的106例胎盘植入疑似病例为研究对象,对孕妇行产前MRI检查与超声检查。对比超声检查与MRI检查诊断灵敏度、特异度、符合率,以及超声与MRI对于胎盘植入诊断的征象。结果超声检查的诊断敏感度、特异度、符合率分别为90.16%、84.44%、87.74%,MRI检查的诊断灵敏度、特异度、符合率分别为93.44%、86.67%、90.57%,两组数据比较差异无统计学意义(χ^(2)=0.436、0.090、0.439,P>0.05)。胎盘植入孕妇胎盘液性暗区、胎盘后间隙异常、胎盘下血管增多及子宫局限性膨隆超声征象的检出率均达80%以上,高于胎盘正常孕妇,差异有统计学意义(χ^(2)=35.049、39.745、42.652、59.732,P<0.05);胎盘植入孕妇胎盘与肌层分界模糊、T2WI低信号带、胎盘信号不均、子宫局限性膨隆MRI征象的检出率均达85%以上,高于胎盘正常孕妇,差异有统计学意义(χ^(2)=29.850、50.435、69.019、69.807,P<0.05)。结论产前超声检查与MRI检查均对胎盘植入有较高诊断价值,其诊断敏感性良好,临床可视患者情况合理进行选择,MRI检查可作为超声检查的辅助和补充。
Objective To analyze the diagnostic value of prenatal ultrasound and MRI for placental accreta.Methods The 106 cases of suspected placenta accreta admitted in the hospital from October 2017 to October 2019 were conveniently selected as the study objects,prenatal MRI and ultrasound examinations were performed on pregnant women.Compare the diagnostic sensitivity,specificity,and coincidence rate of ultrasound and MRI,as well as the signs of ultrasound and MRI for the diagnosis of placenta accreta.Results The diagnostic sensitivity,specificity,and coincidence rates of ultrasound examination were 90.16%,84.44%,and 87.74%,respectively,the diagnostic sensitivity,specificity,and coincidence rates of MRI examination were 93.44%,86.67%,and 90.57%,respectively,there were no statistically significant difference between the two groups of data(χ^(2)=0.436,0.090,0.439,P>0.05).The detection rate of placental fluid dark zone,abnormal retroplacental space,subplacental blood vessel increase and localized uterine bulging in pregnant women with placenta accreta reached 80%and above,which were higher than that of pregnant women with normal placenta,and the difference were statistically significant(χ^(2)=35.049,39.745,42.652,59.732,P<0.05);pregnant women with placenta accreta blur the boundary between placenta and myometrium,low signal band on T2WI,uneven placental signal,the detection rate of MRI signs of localized uterine bulge were over 85%,which were higher than that of pregnant women with normal placenta,and the difference were statistically significant(χ^(2)=29.850,50.435,69.019,69.807,P<0.05).Conclusion Both prenatal ultrasound examination and MRI examination have high diagnostic value for placental implantation,and their diagnostic sensitivity is good.Clinically,patients can make reasonable choices based on the patient's condition.MRI examination can be used as an auxiliary and supplement to ultrasound examination.
