摘要
目的研究不同抗苗勒管激素(AMH)水平对接受体外受精-胚胎移植(IVF-ET)治疗患者卵巢反应性的预测价值。方法选取2019年2月至2020年2月衡水市人民医院诊断为不孕症并接受IVF-ET治疗的150例患者作为研究对象。根据AMH水平分为高水平组、中水平组、低水平组,评估AMH水平对患者卵巢反应性的预测价值。结果低水平组年龄最大,中水平组基础黄体生成素(LH)水平最低,高水平组基础卵泡刺激素(FSH)水平最低、窦卵泡计数(AFC)水平最高、移植胚胎数与获卵数最多,差异具有统计学意义(P<0.05)。月经期AMH(AMH1)、降调后AMH(AMH2)及扳机日AMH(AMH3)、AMH1~AMH3预测卵巢反应性的特异度、灵敏度均较高(P<0.05)。结论 AMH1~AMH3、AMH1、AMH2及AMH3对卵巢反应性具有较高的预测能力,其可作为接受IVF-ET治疗患者卵巢反应的血清学标记物。
Objective To investigate the predictive value of different AMH on ovarian response in patients receiving IVF-ET treatment. Methods 150 patients diagnosed with infertility and treated with IVF-ET in Hengshui People′s Hospital from February 2019 to February 2020 were selected as the study subjects. According to AMH level, the patients were divided into high level group, medium level group and low level group to evaluate the predictive value of AMH level on ovarian reactivity. Results The low level group had the oldest age, the middle level group had the lowest basal LH level, and the high level group had the lowest basal FSH level and the highest AFC level, and the number of embryos transferred and eggs obtained was the largest, with statistically significant differences(P<0.05). The specificity and sensitivity of AMH(AMH1) during menstrual period, AMH(AMH2) after downmodulation, AMH(AMH3) on trigger day and AMH1-AMH3 were all higher in predicting ovarian response(P<0.05). Conclusions AMH1-AMH3、AMH1、AMH2 and AMH3 have high predictive ability for ovarian response, which can be used as serological markers of ovarian response in patients receiving IVF-ET.
