摘要
目的:探讨川崎病(KD)合并冠状动脉病变(CAL)患儿前白蛋白(PA)、胱抑素C(CysC)、血小板四项变化及临床意义。方法:选取KD患儿38例作为KD组,KD合并CAL患儿42例作为KD合并CAL组。选取同期接受体检的健康儿童40例作为对照组。比较三组PA、CysC、血小板计数(PLT)、血小板平均体积(MPV)、血小板比容(PCT)、血小板分布宽度(PDW)水平。对KD组和KD合并CAL组患儿采用原田危险评分(Harada评分)和小林评分(Kobayashi评分)进行疾病风险评分,并分析PA、CysC、PLT、MPV、PCT、PDW水平与Harada评分、Kobayashi评分的相关性。结果:KD合并CAL组和KD组患儿PA水平明显低于对照组,CysC水平明显高于对照组(均P<0.05)。KD合并CAL组患儿PA水平明显低于KD组,CysC水平明显高于KD组(均P<0.05)。KD合并CAL组和KD组患儿PLT、MPV、PCT、PDW水平高于对照组,且KD合并CAL组高于KD组(均P<0.05)。Pearson相关性分析结果显示,PA水平与Harada评分、Kobayashi评分均呈负相关,CysC和血小板四项水平与Harada评分、Kobayashi评分均呈正相关(均P<0.05)。结论:KD合并CAL患儿CysC、血小板四项水平明显升高,PA水平明显降低,对PA、CysC、血小板四项的检测有助于评估患儿病情。
Objective:To investigate the changes and clinical significance of prealbumin(PA),cystatin C(CysC)and four parameters of platelet in Kawasaki disease(KD)children with coronary arterial lesions(CAL).Methods:38 KD children were selected as the KD group,and 42 KD children with CAL were selected as the KD combined CAL group.Forty healthy children who underwent physical examination during the same period were selected as the control group.The three groups of PA,CysC,platelet count(PLT),mean platelet volume(MPV),plateletcrit(PCT),platelet distribution width(PDW)levels were compared.The Harada score and Kobayashi score were used for disease risk scores for children in the KD group and the KD combined CAL group,and the correlation between the levels of PA,CysC,PLT,MPV,PCT,PDW and Harada score and Kobayashi score were analyzed.Results:The PA level of children in the KD combined CAL group and the KD group was significantly lower than that of the control group,and the CysC level was significantly higher than that of the control group(all P<0.05).The PA level of children in the KD combined CAL group was significantly lower than that of the KD group,and the CysC level was significantly higher than that of the KD group(all P<0.05).The levels of PLT,MPV,PCT and PDW in the KD combined CAL group and KD group were higher than those of the control group,and the KD combined CAL group was higher than the KD group(all P<0.05).Pearson correlation analysis showed that PA level was negatively correlated with Harada score and Kobayashi score,and the levels of CysC and four parameters of platelet were positively correlated with Harada score and Kobayashi score(all P<0.05).Conclusion:The levels of CysC and four parameters of platelet in KD children with CAL are significantly increased,and the level of PA is significantly reduced.The detection of PA,CysC and four parameters of platelet is helpful to assess the condition of the children.
