摘要
目的探讨妇科低年级住院医师规范化培训(简称住培)中,实施妇科内镜手术技能培训与考核体系(gynecological endoscopic surgical education and assessment,GESEA)培训的效果。方法选取首都医科大学附属北京朝阳医院妇科2020年6月至2020年12月参加GESEA培训的低年级(第1至第2年)住院医师共计32人,通过进行模块化宫、腹腔镜模拟训练来提升住院医师的妇科内镜操作技能。通过对比培训前摸底测试、培训后结业考核前后成绩,分析GESEA在低年级住院医师内镜操作培训中的可行性和应用价值。结果住院医师32人接受培训前后初级宫腔镜技能培训与考核模块的Camera Navigation测试与考核成绩为[(96.53±16.96)s比(76.41±15.90)s],Hand-Eye Coordination的测试与考核成绩为[(163.80±12.37)s比(124.70±15.58)s],差异均有统计学意义(均P<0.01)。培训前后初级腹腔镜技能培训与考核模块的Camera Navigation的测试与考核成绩为[(93.72±16.00)s比(73.63±13.56)s],Hand-Eye Coordination的测试与考核成绩为[(159.70±12.93)s比(122.20±17.41)s],Bi-Manual Coordination的测试与考核成绩为[(148.70±10.47)s比(103.00±11.63)s],差异均有统计学意义(均P<0.01)。接受培训前后初级缝合打结训练与考核模块的测试与考核成绩为[(874.00±121.00)s比(605.00±102.00)s],P<0.01,在缝合板的5个打结中,标准打结平均数从2个升至4个,5个缝合中标准缝针平均数从3个升至5个,缝合板破损数从2个降至0个,培训前后有明显提高。结论GESEA培训系统在低年级住院医师妇科内镜操作培训中可在短时间内通过模块化培训提升宫、腹腔镜基础操作技巧和能力。
Objective To investigate the effect of GESEA laparoscopic training in the standardized training of junior residents in the Gynecology Department of Beijing Chao-yang Hospital,Capital Medical University.Methods A total of 32 junior residents from the Department of Gynecology of Beijing Chao-yang Hospital affiliated to the Capital Medical University,received GESEA training from 2020 June to December.The residents’gynecological endoscope operation skills were improved through modular laparoscopic simulation training.The baseline test before training and the graduation examination after training were carried out.The feasibility and application value of GESEA in endoscopic operation training for junior residents were analyzed by comparing the results before and after training.Results The test and examination results of Camera Navigation of HYSTT1 for 32 students before and after training were[(96.53±16.96)s vs(76.41±15.90)s],and Hand-eye Coordination test and assessment scores were[(163.80±12.37)s vs(124.70±15.58)s],with statistically significant differences(all P<0.01).The test and assessment results of LASTT 1 Camera Navigation were[(93.72±16.00)s vs(73.63±13.56)s]before and after training.The test and assessment results of Hand-eye Coordination were[(159.70±12.93)s vs(122.20±17.41)s],and the test and assessment results of Bi-Manual Coordination were[(148.70±10.47)s vs(103.00±11.63)s].The differences were all statistically significant(all P<0.01).The test and assessment results of SUTT 1 before and after training were[(874.00±121.00)s vs(605.00±102.00)s](P<0.01).The average of standard knots in the 5 suture plates increased from 2 to 4,the average of standard stitches in the 5 suture plates increased from 3 to 5,and the number of damaged suture plates decreased from 2 to 0,showing a significant improvement after the training.Conclusion The GESEA training system has a good application value in the gynecological endoscopic operation training of junior residents with improvements in the basic operation sk
