摘要
目的:研究股骨近端锁定钢板(PFLP)与股骨近端防旋髓内钉(PFNA)两种内固定手术方法治疗股骨粗隆间骨折(IFF)老年骨质疏松(OP)患者的效果。方法:2014年1月-2019年12月收治IFF老年OP患者108例,根据治疗方式不同分为两组。对照组应用PFLP治疗;试验组应用PFNA治疗。比较两组患者围手术期相关指标水平、髋关节功能(Harris评分)及术后不良反应情况。结果:试验组手术平均失血量、手术平均X线透视次数、VAS评分、平均手术时间及术后平均负重时间均低于对照组,差异有统计学意义(P<0.05)。两组术后0.5年和术后1年Harris评分均较术前升高,差异有统计学意义(P<0.05);两组术后0.5年和术后1年Harris评分比较,差异无统计学意义(P>0.05)。试验组术后并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:与PFLP相比,PFNA治疗IFF老年OP患者可明显减少手术平均失血量、手术平均X线透视次数,显著控制患者手术后的疼痛感及不良反应,同时可缩短手术时间。
Objective:To study the effects of proximal femoral locking plate(PFLP)and proximal femoral anti rotation intramedullary nail(PFNA)on elderly patients with osteoporosis(OP)and femoral intertrochanteric fracture(IFF).Methods:From January 2014 to December 2019,108 elderly OP patients with IFF were treated and divided into two groups according to different treatment methods.The control group was treated with PFLP.The experimental group was treated with PFNA.We compared the levels of perioperative related indexes,hip function and postoperative adverse reactions between the two groups.Results:In the experimental group,the average blood loss,average X-ray fluoroscopy times,VAS score,average operation time and average postoperative weight-bearing time in the experimental group were lower than those in the control group,the difference was statistically significant(P<0.05).The Harris scores of the two groups at 0.5 and 1 year after operation were higher than those before operation,the difference was statistically significant(P<0.05).There was no significant difference in Harris score between the two groups at 0.5 and 1 year after operation(P>0.05).In the experimental group,the incidence of postoperative adverse reactions was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion:Compared with PFLP,PFNA in the treatment of elderly patients with osteoporotic IFF can significantly reduce the average blood loss and the average number of X-ray fluoroscopy,significantly control the postoperative pain and adverse reactions,and shorten the operation time.
