摘要
目的系统评价穴位按压腕带作用于内关穴对缓解成年女性妇科术后胃肠道不适的作用与效果。方法计算机检索PubMed、Embase、Cochrane、Proquest、中国期刊全文数据库(CNKI)、中国生物医学文献数据库(CBM)、维普和万方数据库,收集国内外相关的临床随机对照试验(RCT),检索时限为建库至2020年12月,采用RevMan 5.3软件进行Meta分析。结果共纳入8个RCT 910例患者。Meta分析结果显示,与安慰剂组相比,试验组穴位按压腕带作用于内关穴可以有效减少妇科术后呕吐的发生(OR=0.53,95%CI:0.30~0.95,P<0.05)与止吐药使用率(OR=0.32,95%CI:0.22~0.47,P<0.00001)。而对于术后恶心的发生率,试验组和安慰剂组比较,差异无统计学意义(OR=0.70,95%CI:0.47~1.04,P>0.05)。结论穴位按压腕带作用于内关穴可以有效降低妇科术后患者呕吐的发生率和止吐药的使用率,而对改善恶心效果不显著,尚需大样本、高质量的临床随机对照试验予以验证。
Objective To systematically evaluate the effect and efficacy of acupressure wristbands applied to the Neiguan acupoint on relieving gastrointestinal discomfort in adult females after gynecological operation.Methods A variety of databases such as PubMed,Embase,Cochrane,Proquest,China Academic Journals Full-text Database(China) National Knowledge Infrastructure(CNKI),China Biology Medicine(CBM),Vip and Wanfang databases were searched by computer,and relevant clinical randomized controlled trials(RCT)at home and abroad were collected.The search term was from the establishment of the databases to December 2020.The meta-analysis was performed using RevMan 5.3 software.Results A total of 8 RCT and 910 patients were included in this analysis.The Meta-analysis results showed that compared with the placebo group,the incidence of vomiting(OR=0.53,95%CI:0.30~0.95,P<0.05)and the use of antiemetic drugs(OR=0.32,95%CI:0.22~0.47,P<0.00001)after gynecological operation effectively reduced in the trial group,namely,the group with acupressure wristbands applied to the Neiguan acupoint.In terms of the incidence of postoperative nausea,there was no statistically significant difference between the trial group and the placebo group(OR=0.70,95%CI:0.47~1.04,P>0.05).Conclusion Acupressure wristbands applied to the Neiguan acupoint can effectively reduce the incidence of vomiting and the use of antiemetic drugs in patients after gynecological operation,but the effect on improving nausea is not significant and needs to be verified by high-quality randomized controlled clinical trial with large sample size.
