摘要
目的通过数据挖掘分析探讨清代著名医家叶天士治疗耳鸣用药特点及思路,为临床耳鸣治疗提供参考。方法检索并整理古今医案云平台中的叶天士治疗耳鸣医案,采用系统集成的统计分析、聚类分析、关联规则、复杂网络分析等方法,分析医案的组方用药频次、药性、归经、药物配伍关联等。结果最终纳入46个医案。中药频次结果显示共得到高频中药15种,包括茯神、熟地、磁石等,药物多为寒、平、甘、苦,归肝、心经者;药物聚类分析结果显示,耳鸣的治疗用药主要分为健脾、补阴、镇摄清热4类;关联规则发现常用的中药组合是由茯神、麦冬、甘草3味药组成健脾益胃等功效的药对;复杂网络分析获得使用的核心方药组成是以四物汤为基础加减。结论叶天士治疗耳鸣重视心、肾、胆三脏,善用轻宣之品清热,甘寒之品益胃阴,重镇及补益之品镇摄;核心方药以生地、熟地、牡蛎、茯神、连翘、白芍、羚羊角、磁石、天冬为主,最常用单味中药为:茯神、熟地黄、磁石等;组合用药则以茯神、麦冬、甘草3味药组多见。
Objective To investigate the characteristics and ideas of Ye Tianshi, a famous doctor in Qing Dynasty,in the treatment of tinnitus by data mining analysis, so as to provide reference for clinical tinnitus treatment.Methods The medical cases of Ye Tianshi in treatment of tinnitus in ancient and modern medical case cloud platform were searched and sorted out. The system-integrated statistical analysis, cluster analysis, association rules and complex network analysis were used to analyze the frequency of drug use, medicinal properties, meridian return and drug compatibility association of medical cases. Results 46 medical cases were included. The frequency results of traditional Chinese medicine showed that a total of 15 kinds of high-frequency traditional Chinese medicine were obtained, including Poria, prepared Radix Rehmanniae, magnet, etc. And most of the drugs were cold, smooth, sweet, bitter and those returned to liver and heart scriptures. The results of drug cluster analysis showed that the therapeutic drugs for tinnitus were mainly divided into three categories: strengthening spleen,tonifying Yin and clearing heat. Association rules found that the commonly used traditional Chinese medicine combination is composed of Poria, Ophiopogon japonicus and licorice, which can strengthen spleen and nourish stomach. The core prescription composition obtained by complex network analysis was based on Siwu decoction.Conclusion Ye Tianshi pays attention to the heart, kidney and gallbladder in the treatment of tinnitus, and makes good use of the mild products to clear away heat, the sweet and cold products to benefit the stomach and Yin, and the important and tonic products to calm the mind. The core prescriptions are mainly Radix Rehmanniae, prepared Radix Rehmanniae, oysters, Poria, Forsythia suspensa, Radix Paeoniae Alba, antelope horn, magnet and asparagus.The most commonly used single traditional Chinese medicines are Poria, prepared Radix Rehmanniae, magnet,etc. The combination of Poria, Ophiopogon japonicus and lico
