在读住院医师规范化培训学生对循证医学的认知、态度、需求及影响循证医学实践原因的调查 被引量:2

摘要 目的调查在读住院医师规范化培训学生(规培生)对循证医学的认知、态度、需求,以及影响其循证医学实践的原因。方法根据相关文献、预调查结果及相关领域专家意见,自制问卷调查表。随机抽取江西省部分国家规培基地在读规培生400名,调查其基本情况,对循证医学的认知、态度、需求,以及其认为阻碍循证医学实践的原因。结果 (1)共发放问卷400份,回收有效问卷395份,有效回收率98.75%。(2)大部分规培生认为循证医学对指导临床很有帮助(占81.01%),并认为循证医学可实现的最主要作用为指导临床实践(占90.38%)和提高临床业务能力(占79.24%)。分别有24.3%(96/395)、56.20%(222/395)、32.66%(129/395)的规培生对循证医学、循证医学新九级证据、最新医学信息不了解(包括不太了解、很不了解、完全不了解)。101名(25.57%)规培生认为专家共识是最可靠的医学证据。(3)372名(94.18%)规培生认为临床实践中运用循证医学非常必要,370名(93.67%)愿意在临床实践使用循证医学。(4)286名(72.40%)规培生认为在临床实践中应以循证医学为主。(5)95.70%(378/395)的规培生认为有必要开设循证医学培训课程,75.70%的(299/395)规培生愿意参加循证医学培训。(6)缺少循证医学相关知识(16.63%)、缺少时间(16.49%)、外文文献阅读能力不足(12.56%)是阻碍规培生循证医学实践的主要原因。结论部分在读规培生对循证医学了解程度不足,缺少循证医学知识和时间、外文阅读能力不足阻碍了其循证医学实践,但多数在读规培生认为有必要引进且愿意参加循证医学培训。 Objective To investigate the cognition,attitude,and demand related to evidence-based medicine(EBM),as well as the reasons influencing the practice of EBM among in-school students receiving standardized resident training(trained students).Methods A self-made questionnaire was developed based on the relevant documents,pre-survey results and experts′opinions in related fields.A total of 400 in-school trained students were randomly selected from several national standardized resident training bases in Jiangxi Province,among which a survey was conducted on their general information,cognition,attitude and demand related to EBM,and the reasons which were believed to hamper the practice of EBM.Results(1)A total of 400 questionnaires were sent out,395 validated were returned,and the effective recovery rate was 98.75%.(2)The majority(81.01%)of trained students believed that EBM was very helpful for clinical guidance,and also considered that the most common effects which EBM could exert included providing guidance for clinical practice(90.38%)and improving the clinical professional proficiency(79.24%).About 24.3%(96/395),56.20%(222/395),and 32.66%(129/395)of trained students did not know(not well know,poorly know,or know nothing)about EBM,EBM level 9 of evidence,and up-to-date medical information,respectively.Totally 101(25.57%)trained students considered expert consensus the most reliable medical evidence.(3)Totally 372(94.18%)trained students considered that it was quite necessary to apply EBM to clinical practice,and 370(93.67%)were willing to apply EBM to clinical practice.(4)A total of 286(72.40%)trained students thought that clinical practice should be dominated by EBM.(5)About 95.70%(378/395)of trained students considered offering the course of EBM training necessary,75.70%(299/395)were willing to participate in EBM training.(6)Lack of knowledge about EBM(16.63%),inadequate time(16.49%),and low ability for foreign documents reading(12.56%)were the main reasons hampering the practice of EBM in trained students.Concl
