摘要
目的:研究在产后乳房肿胀中应用徒手淋巴引流结合乳房按摩的效果。方法:选取2019年6月-2020年6月产后1~3 d发生Ⅱ度及以上乳房肿胀的产妇100例,随机分为两组,各50例。对照组采取乳房按摩+常规哺乳指导,观察组采取徒手淋巴引流+乳房按摩+常规哺乳指导,比较两组干预效果。结果:观察组干预6 h、12 h、24 h、48 h、72h后的纯母乳喂养率均高于对照组,差异有统计学意义(P<0.05);观察组干预6 h、12 h、24 h、48 h、72 h后的数字疼痛分级(NRS)评分及乳房肿胀程度均低于对照组,差异有统计学意义(P<0.05);观察组干预后12 h、24 h无或轻度疼痛率均高于对照组,差异有统计学意义(P<0.05)。结论:产妇产后乳房肿胀采取徒手淋巴引流结合乳房按摩,可减轻产妇产后乳房疼痛与肿胀程度,增加泌乳量,增加母乳喂养率,促进其产后恢复。
Objective:To study the effect of freehand lymphatic drainage combined with breast massage in postpartum breast swelling.Methods:From June 2019 to June 2020,100 cases of parturients with breast swelling of degreeⅡand above in 1 to 3 days after delivery were selected,they were randomly divided into the two groups with 50 cases in each group.The control group was given breast massagea and routine breastfeeding guidance.The observation group adopted freehand lymphatic drainage and breast massage and routine breastfeeding guidance.We compared the intervention effects of the two groups.Results:The exclusive breastfeeding rate of the observation group after 6 h,12 h,24 h,48 h and 72 h of intervention were higher than that of the control group,the differences was statistically significant(P<0.05).The NRS score and breast swelling degree of the observation group after 6 h,12 h,24 h,48 h and 72 h of intervention were lower than those of the control group,the differences was statistically significant(P<0.05).The rates of no or mild pain at 12 h and 24 h after intervention in the observation group were higher than those in the control group,the differences was statistically significant(P<0.05).Conclusion:Freehand lymphatic drainage combined with breast massage in the treatment of postpartum breast swelling of the parturient can reduce the pain and swelling of the breast,increase the lactation,increase the breastfeeding rate,and promote the postpartum recovery.
