摘要
目的:探讨蛛网膜囊肿腹腔分流管移位原因、发病机制及预后。方法:回顾性分析本院2019年11月因左侧额顶部蛛网膜囊肿行囊肿腹腔分流术,术后分流管自行脱出颅外并痊愈的案例,结合文献复习讨论蛛网膜囊肿腹腔分流术后分流管移位的处理及预后。结果:患儿蛛网膜囊肿消失痊愈,临床症状改善,拔除移位的分流管,文献复习蛛网膜囊肿腹腔分流管移位并痊愈罕见,主要为分流管远端移位多见,坠入阴囊、脱出肛门以及移位至胸腔,甚至心脏,需要再次手术处理,及时处理预后良好。结论:蛛网膜囊肿腹腔分流术可能是治疗儿童大脑凸面蛛网膜囊肿的有效方法,也提示大脑凸面蛛网膜囊肿消失后可以拔除分流管,分流管移位临床罕见,需要及时手术处理。
Objective:To investigate the causes,pathogenesis and prognosis of shunt displacement in arachnoid cyst.Methods:Retrospective analysis was made on the case of postoperative shunt self-prostrated and recovered due to the left frontal top arachnoid cyst undergoing cyst-peritoneal shunt in our hospital in November 2019.Combined with literature review,the treatment and prognosis of shunt displacement after arachnoid cyst peritoneal shunt were discussed.Results:The children patients were recovered,with disappearance of arachnoid cyst,improvement of clinical symptoms and pulling out of the displaced shunt tube.By literature review,it was found that cases of arachnoid cyst cured were rare when arachnoid cyst peritoneal shunt tubes were displaced.The shunt tube displacement often took place in distal places,such as falling in the scrotum,extricating from the anus,falling into the thorax or even the heart.When the shunt tube displacement took place,it required reoperation and management timely,so that good prognosis could be achieved.Conclusion:Arachnoid cyst peritoneal shunt may be an effective method for the treatment of cerebral convex arachnoid cyst in children,and it also suggests that the shunt tube can be removed after the disappearance of cerebral convex arachnoid cyst.Shunt tube displacement is rare in clinic and needs timely surgical treatment.
